Facelift surgery is the most powerful procedure for facial skin sagging and offers the longest-lasting results, but it is not easy to decide on surgery because of the recovery period and the burden of incisions.
Lifting procedures that use ultrasound or radiofrequency, as well as skin tightening with Botox and skin boosters, are also good options. However, if the lifting effect of these procedures is insufficient, you may want to consider thread lifting using lifting threads.
'The Two Types of SMAS Used in Thread Lifting'
The most important tissue to consider in facial lifting, including facelift surgery, is the SMAS layer.

The facial SMAS layer can be divided into Fixed SMAS and Mobile SMAS.
The Fixed SMAS is a firm, tough SMAS tissue that does not move easily unless it is separately dissected in a facelift. In contrast, the Mobile SMAS is a soft, movable tissue that is dissected and pulled when the SMAS layer is tightened in a facelift.
So in a facelift, when dissecting the SMAS layer, the Mobile SMAS is dissected and fixed to the firm Fixed SMAS side. This same principle can also be used in thread lifting.

In thread lifting, lifting threads with barbs are used, and threads of various lengths and thicknesses are selected depending on the area of the face being lifted.
Because the barbs on lifting threads are arranged in both directions, they can create a fixation point for the thread above and lift tissue below.
An important point in thread lifting is that the cannula containing the thread must pass through the Fixed SMAS and enter the Mobile SMAS.
Through this process, a fixation point is created on the firm Fixed SMAS, and this fixation point can be used to pull the sagging facial tissue.

Thread lifting has the advantages of producing immediate results, causing little swelling or bruising, and having a short recovery period.
Although thread lifting is a relatively simple procedure, it is important for the practitioner to have a good understanding of facial anatomy and insert the threads into the correct layer.
Rather than simply increasing the number of threads to enhance the lifting effect, it is more important to create a firm fixation point in the Fixed SMAS area of the facial SMAS layer and to pull the Mobile SMAS.
Only when the lifting threads are inserted into the correct layer can thread-lifting-related side effects such as dimpling be prevented and the lifting effect be maintained well.
