When counseling patients about eye plastic surgery, I often see cases where multiple problems appear together rather than just one isolated issue.
Especially after middle age, drooping eyelid skin is often accompanied by hollowing in the upper eyelids or ptosis.

First, blepharoptosis refers to insufficient strength to open the eyes, regardless of skin laxity.
The procedure used to address this is called ptosis correction surgery, or cosmetic eyelid-opening surgery when performed for aesthetic purposes.
Simply put, it may be helpful to understand “cosmetic ptosis correction surgery” = “cosmetic eyelid-opening surgery.”
In addition, when a problem from a previous double-eyelid surgery was performed incorrectly or was not fully resolved, the overall condition often becomes even more complex.

This case involved a woman in her early 60s who had
asymmetry between the left and right double-eyelid folds,
moderate ptosis,
and fullness in the upper eyelids and the area around the bridge of the nose.
When several problems are present, trying to resolve everything with just one surgery may lead to lower satisfaction.
The important step is to analyze the causes separately rather than focusing only on the visible symptoms.
First, the asymmetry of the double-eyelid folds and the ptosis were problems originating in the eyes themselves.

Because there was a difference in the height of the double-eyelid folds and the eyes did not open with sufficient strength,

This helped balance the double-eyelid lines on both sides and improve the function of opening the eyes.

However, there was one more factor that needed to be considered.
When the upper eyelids look full, this is not necessarily caused by an excess of fat in the upper eyelids.
In many cases, as people age, the tissues of the forehead descend, pressing the tissues above the eyes downward.

If only double-eyelid surgery and cosmetic eyelid-opening surgery are performed in this condition, the strength of eye opening may improve, but the fullness of the upper eyelids may instead appear more pronounced in comparison.
Therefore, in this case, endoscopic forehead lifting was performed together with the eye procedures described above.

Using an endoscope to elevate the forehead, eyebrows, and tissues above the eyes to an appropriate position can improve the heavy fullness around the upper eyelids and bridge of the nose.
Seeing how lengthy the explanation is may make the consultation and surgery seem somewhat complicated.
However, when multiple problems overlap, approaching each cause separately rather than trying to resolve everything with a single surgery can lead to a more natural and satisfactory result.

At five months after surgery, the photographs show that the asymmetry between the double-eyelid folds had improved considerably, and that the eyes appeared much more comfortable as the strength of eye opening improved.
The forehead lift also improved the fullness around the upper eyelids and bridge of the nose, creating an overall impression that looked more open and bright.
Eye plastic surgery is not simply a procedure for creating a single double-eyelid line. It is important to consider the entire structure surrounding the eyes.
Especially when several problems exist at the same time, identifying each cause and addressing those causes appropriately can determine the result.
