Structural Improvement of N-Side Fold Lines in Sinsa Using N-Scaffold | 2025 PENTA-A Lecture Review

Hello, I am Director Jea Wook Jeon of Lifton Dermatology Clinic.

Last November, the 2025 PENTA-A Anti-Aging and Aesthetic Anatomy Research Society conference was held.

This conference focused on understanding aging based on anatomy and its practical clinical application, and it was a place where in-depth discussions were held on how various skin aging treatments, thread lifting, fillers, regenerative treatments, and energy-based devices should be approached structurally.

At this conference, I gave a lecture on “Structural Improvement of Nasolabial Folds Using N-Scaffold.”

These days, as I treat patients directly, I often meet many people concerned about nasolabial folds in Sinsa.

For that reason, this lecture was meaningful not just as an introduction to a treatment method, but as an opportunity to organize and share clinical criteria.

What is the problem with nasolabial folds in Sinsa?

The first point I discussed in the lecture was that

a nasolabial fold is not a surface skin problem, but a structural change.

As the fat pad located below the cheekbone moves downward, the supporting structure weakens, and the ligaments are no longer able to support the volume.

When this is combined with repeated facial expressions, muscle pull, loss of mid-layer volume, and reduced elasticity of the superficial layer, a “line-like nasolabial fold” naturally forms.

If filler is injected while ignoring these complex causes, it may look fine from the front, but when smiling it can appear heavy or the area around the mouth may look unnaturally fixed.

That is one of the biggest concerns for patients.

Why N-Scaffold?

N-Scaffold is one of the treatment methods that has recently been drawing attention in clinical practice.

Unlike ordinary threads, its 12-line-based, 24-strand multi-braided structure does not easily lose its shape over time, and it naturally creates a sense of integration with the tissue.

The biggest change I felt while using this product was that it did not feel like forcibly filling in volume, but rather like unfolding a folded structure.

That is why, in nasolabial fold cases, it is possible to directly feel the change: it is not obvious, yet it naturally softens along with facial expressions, which is an important factor.

The treatment is determined not by how much is placed, but by where it is placed.

The topic that received the most questions during the lecture was the layer in which N-Scaffold should be applied.

A nasolabial fold in Sinsa is not something that can be resolved in just one layer, but rather the result of structural changes occurring simultaneously in multiple layers.

Therefore, treatment is also more natural and stable when it is not approached by handling only one layer at a time, but by organizing the layers in sequence.

First, the space and boundaries that have shifted downward in the deepest layer are released to relieve structural tension. Then, if necessary, volume in the mid-layer is supplemented to rebuild the collapsed contour. Finally, folds created by repeated facial movements in the superficial layer are organized and the supporting structure is stabilized.

This approach creates a change that is completely different from simply filling in volume.

It may seem subtle in a single frontal photo, but the biggest difference appears when facial expressions move and the flow of the nasolabial line changes naturally.

And that difference is what creates results that are not obvious yet last for a long time.

The result appears more in facial expression than in the front view.

The most interesting thing after applying N-Scaffold is that the difference becomes visible in motion.

The subtle changes that were felt in the resting state are

clearly seen as the folding strength of the nasolabial line is eased when smiling or speaking.

There are many patients who say, “I’m not sure what changed, but my expression feels more relaxed.” That difference is exactly the boundary between a natural improvement and a treatment that looks obvious.

Natural results come from understanding the structure.

What I was able to confirm again while preparing and delivering this conference presentation is that treatment for nasolabial folds ultimately must be based on an accurate understanding of anatomical structure before selecting the material.

N-Scaffold is an effective tool within that structural approach.

By itself, it does not create the result, but when applied at the appropriate layer, with the correct direction and depth, it provides stability and naturalness that are difficult to achieve by other methods.

Ultimately, the important question is not “What should be used?”

It is “Can we explain why that material should be placed in that location?”

That standard of judgment should be based not on experience alone, but on anatomical evidence and clinical observation.

This lecture was meaningful because it helped organize and share those standards.

Going forward, rather than simply trying new procedures,

I will continue to accumulate clinical data so that the approach to treating nasolabial folds in Sinsa becomes more structural and more predictable,

and I will continue the process of academically verifying those results.
