[Eyelid Revision Surgery] Failure Cause Analysis & Customized Solutions

Eyelid surgery is a procedure many people choose,

but many also consider revision surgery.

As double eyelid surgery and canthoplasty have become more common,

the frequency of revision surgery has increased even more,

and in some cases, it even continues to a third or fourth revision.

Unfortunately, revision surgery is much more complex and difficult

than the first operation.

Therefore, an accurate analysis of the cause

and a customized solution tailored to it are essential.

Let’s take a closer look at common types of eyelid revision surgery

and how they are resolved.

Revision surgery: why is it necessary?

Analysis of common failure causes

Sausage eyes (excessively large crease)

Cause: This occurs when the double eyelid crease is made too large,

over 10 mm, or when tissues become severely tangled after frequent surgeries.

Problem: It creates an unnatural and awkward impression,

and in severe cases, it can also affect the function of opening the eyes.

Sunken scar (strong fixation):

Cause: Due to excessive fixation, the tissue becomes severely

tangled and adhered, making the scar appear sunken.

Problem: The eyelid looks unnaturally hollow,

and in severe cases, there may be discomfort when closing the eyes.

Crease loosening:

Cause: This mainly occurs after surgery using the buried suture method

or the dynamic incisional method,

and refers to the double eyelid crease becoming faint or loosening.

Problem: The double eyelid may disappear or the line may become unclear,

making the eyes look heavy.

A crease that is too small or covered:

Cause: This happens when the crease height is set too low, under 5 mm,

or when the eyelid skin sags and covers the crease.

Problem: The crease is barely visible, like a hidden double eyelid,

which can create a heavy and sleepy impression.

Revision surgery: how is it solved?

Customized solutions

Reducing a large crease:

If there is enough excess skin:

By designing and excising including the existing crease,

this method can achieve the most effective result.

If there is insufficient excess skin:

This method releases the crease by making an incision below it

while preserving the existing crease.

Its drawback is that double creases may form over time.

Making a small crease larger:

If the measured crease is 6–8 mm:

Removing only the overlying skin may be enough to create a more open-looking crease.

If the measured crease is 5 mm or less:

To enlarge the crease, a two-line technique is needed,

and the existing scar line may remain.

A second surgery may be needed to remove the scar.

Restoring a loosened crease:

Revision surgery can be performed using the same method as the first operation

to recreate the crease.

This is a type that can be resolved relatively simply.

Choosing a board-certified plastic surgeon,

Revision eyelid surgery is much more complex

and requires more delicate technique than the first surgery.
