Uneven eyes and ptosis,

As such, left-right asymmetry of the eyes, especially when one eye looks noticeably smaller or more droopy, many patients experience small but persistent inconvenience and stress in daily life.

This may not be a simple cosmetic issue, but rather a functional abnormality, and a representative cause is ptosis.

Causes of ptosis and uneven eyes

When the muscle that opens the eyes (levator palpebrae superioris) is weak

→ the eyelid cannot lift sufficiently and partially covers the pupil.

When symptoms appear in only one eye, it shows up as uneven eyes

→ one eye is normal, while the other looks sleepy or smaller, creating asymmetry

Compensatory action

→ The forehead is unconsciously raised, causing a habit of using the forehead to open the eyes, which deepens forehead wrinkles and may also cause eyebrow asymmetry

Main concerns of patients

• Eye asymmetry appears more prominent in photos or the mirror

• Social self-consciousness because of comments that they look sleepy

• The forehead feels tense from constantly trying to open the eyes

Daily stress from makeup or using double eyelid tape

The reason for hesitating about surgery is

the worry, “Can both sides be matched exactly?”

Accumulated eye strain due to insufficient field of vision

Overuse of the forehead muscles

→ worsening wrinkles and possible tension headaches

Worsening eyebrow asymmetry

As time passes, eyelid drooping may become more severe

Because of the functional problem with the eyes, not only appearance but also

daily fatigue increases

🛠 Surgical method: correcting uneven eyes + ptosis

This method adjusts the length of the muscle that opens the eyes

to balance pupil exposure.

“Generally, shortening the muscle by 4 mm

results in about 1 mm more exposure of the eyeball.”

Actual surgical case

Right eye: weak opening strength → muscle shortened by 8 mm

Left eye: relatively normal → muscle shortened by 4 mm

Result

Balanced the degree of pupil exposure

and the size of the double eyelid crease on both sides

When uneven eyes and ptosis are present together,

a simple symmetry procedure is not enough;

a combined consideration of function and

appearance is necessary.

This is a high-difficulty surgery that requires

experienced skill and delicate adjustment.

Uneven eyes and ptosis are not problems that will “get better if you just endure them.”

After carefully diagnosing the functional and structural differences between both eyes,

the muscle length and position must be individually adjusted

to achieve stable results.

A precise preoperative diagnosis is essential,

Uneven eyes and ptosis should not

be seen simply as a “difference in eye size.”

Because both appearance and function are affected,

an accurate diagnosis and a customized surgical plan are essential.
