What is a “middle-aged eye”?
“Middle-aged eye” is a general term for
age-related changes around the eyes that appear in people in their 40s to 60s.
As eyelid drooping (upper eyelids), bulging under-eye fat,
increased wrinkles, and changes in the eye shape appear together,
it can involve not only cosmetic concerns but also
functional discomfort.

✅ Causes and symptoms of middle-aged eye changes
Causes
Reduced skin elasticity
Weakened eyelid muscles
Changes in the position of the fat layer
Repeated use of the eyes (fatigue, use of the brow muscles, etc.)
Symptoms
The upper eyelids cover the eyes, making them look smaller

- Brow-under incision method (subbrow incision)
This method makes an incision just below the eyebrow to naturally remove only the sagging upper-eyelid
skin.

When the eyebrow position is fine but only the upper-eyelid skin has sagged

- Upper blepharoplasty

It can improve sagging while also reworking the double-eyelid line.
Suitable cases
When upper-eyelid sagging is severe
When the muscles and fat are also a problem
When there is no double-eyelid line at all
- Brow-under incision + upper blepharoplasty (combined)
This method improves skin sagging with a brow-under incision,
and then addresses the muscles and fat with upper blepharoplasty.
It is recommended when moderate to severe sagging and muscle changes
are present together.
Although the surgical range is wider,
it can improve the overall structure and function of the eyes
in a more harmonious way.

Avoid irritation until the stitches are removed.
Avoid alcohol and smoking

Stitch removal (5–7 days later), swelling checks, and more
