The key difference between female and male double eyelid surgery
For both men and women, the surgical methods
(non-incisional method, incisional method, ptosis correction, etc.) are the same,
but there is a big difference in how the line is designed and in the size setting.
| Item | Female double eyelids | Male double eyelids |
|---|
| Double eyelid height | Usually 7~9 mm | Usually 5~6 mm (set lower) |
| Line design | Strong and clear | Subtle and unobtrusive |
| Impression | Clear and kind-looking | Natural and soft-looking |
| Purpose of surgery | Focused on aesthetic change | Improvement in appearance + functional recovery |

✅ 5 things you must know when choosing male double eyelid surgery
- It is more natural to set the double eyelid line lower.
If a man’s double eyelid line is too high or too pronounced,
it can look awkward.
In most cases, a low line of around 5~6 mm is set,
so that the eyes open clearly
while the double eyelid itself remains inconspicuous.
- The incisional method is often more suitable.
Men often have thicker skin and more eyelid fat,
so with the simple non-incisional method,
the result may be difficult to maintain.
The incisional method or incisional method + ptosis correction
may be appropriate, so an accurate diagnosis is important.
- Eye surgery that does not look obvious is the trend.
For male eye surgery, the ideal result is not so much “creating a double eyelid,” but rather
“the eyes look clearer” and “the eyes look refreshed.”
Natural-looking change is the key.
- If ptosis correction is performed at the same time, it can also improve the field of vision.
If the eyelids droop or you have a habit of using your forehead muscles to open your eyes,
ptosis correction can be performed together
to improve functional discomfort as well.
- The design should match your occupation and lifestyle.
Office workers, civil servants, job seekers, and others
especially prefer a natural and stable impression.
Not only medical judgment but also the social environment
should be considered when deciding on the design.

A note from a board-certified plastic surgeon!
Male double eyelid surgery is
not simply a procedure to create a double eyelid line.
Although the same surgical methods used for women are applied,
men have thicker eyelid skin
and a more developed muscle layer, so the surgical design and
approach differ.
In particular, since men tend to prefer a low and subtle line
rather than a strong, high double eyelid,
setting a shallow line of about 5~6 mm
creates a natural and refined impression.
In many cases, ptosis of the eyelid or the function of the muscles that open the eyes
also needs to be considered, so the incisional method
is often performed together with ptosis correction.
What matters most
is to create a change that looks
“as if you didn’t have surgery.”
A design that takes into account your eye structure and occupational characteristics
is necessary,
and it is most important to decide the direction of surgery
through sufficient consultation with an experienced specialist.
The core of male eye surgery is
to focus on the change in “impression,”
not just the double eyelid.
