“Please make the line bigger.”
“Please make it very thin, almost like a hidden double eyelid.”
During consultations, I often hear requests that are complete opposites like this.
The 기준 for double eyelid line size differs from person to person, and so do preferences.
But in actual surgery, it cannot be determined by desired size alone.
That is because even with the same measurement, different eye structures produce completely different results.


The structure you should understand before deciding on double eyelid line size
To talk about double eyelid line size, you first need to understand a structure called the tarsal plate.
The tarsal plate is a firm, crescent-shaped structure inside the eyelid, and its average height is about 9 mm.
The muscle that opens the eye attaches above the tarsal plate, and the double eyelid crease is also formed in connection with this structure.
So in general, an incision is made at around 7 mm, and fixation needs to be done at the upper part of the tarsal plate so that the eyelashes lift naturally and the so-called “sausage” effect is reduced.
The problem starts here.
Even with the same 7 mm line, the result varies depending on skin thickness, the amount of fat, and how protruded the eyeball is.
This is also why, after a month, the size and shape of the line settle differently from person to person.

If there is significant eyelid drooping, the line should not be made bigger
When the eyelid skin is heavily drooped, the eye may look like it has an inner double eyelid even after surgery at 7–8 mm.
If the line is made even larger in such cases, it may look bigger at first glance, but once it goes beyond the height of the tarsal plate, an awkward crease can easily appear.
In the end, as time passes, it may look thick or resemble a “sausage eye.”
In this case, rather than forcing the line to be larger, it may be more appropriate to properly address the drooping skin.
When the skin is removed, less use of the eyebrow and forehead muscles is needed, and the eyes appear much more defined.
However, if too much of the orbicularis oculi muscle under the skin is removed, a depressed scar can form, so this area must be approached very carefully.

For protruding eyes and sunken eyes, the line should be considered smaller
When the eyeball protrudes, the eyelid is pushed by the eyeball, so the line appears larger than it actually is.
In such cases, if the double eyelid line size is made too large, the unnatural impression can become even stronger.
On the other hand, when the upper eyelid area is sunken, the skin folds inward, making the line look larger, so it is more natural to set it slightly smaller.
In the end, double eyelid line size is a matter of structure.
The same 8 mm can look glamorous in one eye and like an inner double eyelid in another.
If you cannot read this difference, you will end up far from the result you wanted.

Balance is more important than trends in double eyelid line size
During consultations, some people show photos of certain celebrities and ask for the same line.
But if the eye shape, bone structure, and skin condition are different, it cannot be made exactly the same.
When deciding double eyelid line size, you should first consider harmony with your own face rather than trends or photos.
If the line is too large, it can look artificial; if it is too small, it can look heavy.
A naturally clear line is usually determined within the height of the tarsal plate.
If you go beyond this 기준, satisfaction is more likely to decrease over time.

Before surgery, I first analyze the structure of the eyes and suggest a line that suits it.
Rather than changing things too much, it is important to focus on finding the direction that matches the current face.
