When talking with people considering double-eyelid surgery, there is something I hear often.
They say they want a clear line without looking like they had surgery.
In my experience, fewer than 5% of people have all the conditions needed to make that goal possible.
In general, depending on each person, the place, depth, and degree of fixation all need to be carefully set.

Dynamic incision method
The conditions for a natural-looking eye are more demanding than you might think
In theory, there are eyes for which surgery is relatively easy.
These are eyes with long horizontal length, thin skin, good elasticity, and strong eyelid-opening ability.
If those conditions are present, a relatively stable result can be achieved no matter how the fixation is done.
But in reality, it is uncommon for all of these conditions to be present.
Most cases involve at least one factor such as thick skin, sunken eyeballs, protruding eyeballs, or weak eyelid-opening strength.
That is why double-eyelid line fixation cannot simply be explained by the formula that stronger is better and weaker is more natural.
The shape and movement of the eye must be considered together, and the position and strength of fixation must be adjusted accordingly.

Static incision method
Sunken eyes and protruding eyes require different approaches
Eyes with sunken eyeballs and thick skin can easily loosen if the fixation is weak.
In such cases, relatively firm fixation is needed, and the gradual loosening that occurs over time must be taken into account.
However, if the fixation is too strong, it may not loosen and could remain as is, which can look awkward.
The goal of surgery is to find this balance.
On the other hand, the situation is different for eyes with protruding eyeballs.
Because the eyelids are pushed by the eyeballs, the line can appear larger than expected.
At this time, even slightly strong fixation can make the eyes look overly emphasized.
So, it is more natural to set the line smaller and adjust it delicately.
In the end, double-eyelid line fixation requires different interpretation depending on the structure of the eyes.

Dynamic + Static
Dynamic and static: the know-how of designing matters more than the method
There are methods called dynamic and static in incision surgery.
Dynamic looks relatively natural when the eyes are closed, but if the knots are weak, it can loosen.
Static has less risk of loosening, but the fold may remain visible even when the eyes are closed, making the surgery noticeable.
It is difficult to say definitively which method is better.
As a plastic surgery specialist with extensive experience, it is also possible to combine the advantages of both methods.
The key is to design it so that the risk of loosening is reduced while still not looking awkward when the eyes are closed.


The strength of fixation must be balanced carefully
During consultations, people often ask, “Please fix it strongly” or “Please make it naturally weak.”
At that point, the strength of the eyelid-opening muscle, skin thickness, amount of fat, and even the position of the eyeballs all need to be considered.
Even with the same fixation strength, some eyes will loosen while others may look overly pronounced.
That is why we look for the point where the result can remain natural over time.

It should feel comfortable over time
After double-eyelid surgery, the result after 6 months is more important than the appearance right after the procedure.
If the fixation is too strong, awkwardness can remain; if it is too weak, the line may become blurry.
Finding that target point is what comes from a specialist’s experience.
After first analyzing the structure of the eye, the fixation method is decided.
Natural double-eyelid line fixation is not created by chance.
The shape and movement of the eye must be read, and the position and strength of fixation must be finely adjusted.
When that standard is applied, a natural-looking double eyelid is completed.
