What You Should Check Before Rushing to a Ptosis Diagnosis in Revision Eye Surgery

Some people come in after consulting at several places and receiving a ptosis diagnosis.

However, when we look closely at the eyes in person, there are cases where the problem is not the strength of the muscles but the line itself.

A typical example is when the patient had surgery four years ago using the incision method, but the line was made too large, causing a sausage-like appearance and making the eyes look sleepy.

In such cases, the first step is not ptosis correction surgery, but reducing the size of the line.

This is exactly why diagnosis is so important in revision eye surgery.

A larger line does not necessarily make the eyes look bigger

Many people think that the larger the double eyelid line, the more refreshed the eyes will look.

However, this is often a judgment made without considering the structure of the eyes.

Inside the eyelid is a firm structure called the tarsal plate, and the double eyelid line should be designed based on the height of this structure.

If the line is made too high beyond this range, the skin between the eyelashes and the line folds too thickly and looks puffy, like a sausage.

Rather than making the eyes look bigger, it can make them feel compressed and give a sleepy appearance.

Because this is a structural issue rather than swelling, it will not improve on its own over time.

It may not be ptosis

When eyes look sleepy, many people immediately think of ptosis.

But if an excessively large line is interfering with the ability to open the eyes, the problem may not be that the muscle strength itself is weak.

In cases like this one, even if the patient received ptosis diagnoses at several clinics, a direct examination may reveal that the main issue is the size of the line.

If ptosis correction is performed in such a case, it can lead to overcorrection or make the eyes look even more unnatural.

In revision eye surgery, before determining whether ptosis is present, it is important to first check whether the current position and size of the line are designed appropriately for the eye structure.

Reducing the line alone can change the eyes

This case was treated with revision surgery to reduce the line without ptosis correction.

The excessively high line was lowered to an appropriate position, and epicanthoplasty was performed together to improve the cramped appearance of the eyes.

It may seem strange that the line becomes smaller while the eyes look larger.

That is because once the thick line that was pressing down on the eyes is removed, the eyes open more naturally.

When the factors interfering with function are gone, the eyes look clearer and more comfortable.

If epicanthoplasty is performed at the same time, the horizontal length of the eyes can also increase, making the overall impression look even more open.

The result of revision eye surgery is determined at the diagnosis stage

Even eyes that look similarly sleepy can have different causes.

The surgical approach changes completely depending on whether the issue is weak muscle strength, the position of the line, or sagging skin.

If the cause is misjudged and surgery is performed, the same concerns can return even after revision surgery.

In the end, the starting point is to identify the cause precisely through a consultation with an experienced board-certified plastic surgeon.

There are many cases in which someone who has struggled for years finds a natural result after revision eye surgery.
