When Double-Eyelid Surgery Alone Is Not Enough for Creased Folds and Sunken Eyelids: Eye-Opening Correction Surgery

Some people visit us because they are concerned that wrinkles appear layered above their double-eyelid line or that their eyelids look sunken.

On the surface, it may seem that double-eyelid surgery alone would improve the condition, but that is not always the case.

The case I will discuss today is one such example.

I will explain how the results can differ when eye-opening correction surgery is performed alongside double-eyelid surgery for sunken eyelids accompanied by creased folds.

Why do creased folds develop?

Creased folds refer to a condition in which another crease appears folded above the double-eyelid line.

They often develop when the eyelids are sunken or the strength used to open the eyes is weak, causing the skin to fail to rise sufficiently and fold in the middle.

If only double-eyelid surgery is performed in this condition, the line may be created, but the possibility of the creased fold recurring remains.

This is because the force that lifts the eyelid itself has not been strengthened.

For this reason, performing double-eyelid surgery together with eye-opening correction surgery may be a more effective solution.

Why eye-opening correction surgery can improve sunken eyelids

One cause of a sunken-looking eyelid is weakness in the muscle used to open the eye.

When the levator muscle cannot contract sufficiently, the eyelid does not rise properly. The resulting empty space can make the area appear sunken.

By pulling the levator muscle forward and securing it during eye-opening correction surgery, the eyelid can rise further, potentially producing the effect of naturally filling in the area that previously appeared sunken.

Even without a separate fat graft, changing the movement of the muscle alone can alter the shape of the eyelid.

Of course, when the hollowing is significant, a fat graft may also be considered.

Why posterior canthoplasty was performed as well

In this case, posterior canthoplasty was performed along with double-eyelid surgery and eye-opening correction surgery.

When the horizontal length of the eyes is short, the eyes may continue to look constricted even after the eyelids have been refined and the line has been shaped.

Widening the horizontal width of the eyes through posterior canthoplasty changes the overall proportions of the eye area and allows the effects of the preceding procedures to appear more naturally.

When multiple procedures are performed together, it is important to preserve the purpose of each procedure and design them so that their effects work together in balance.

Until the results stabilize after surgery

When double-eyelid surgery and eye-opening correction surgery are performed together, the initial swelling tends to last longer than after a single procedure.

It takes some time for the contours to begin taking shape and for the stable results to become apparent.

Many people are satisfied when they later see that the creased folds have not returned and that the sunken appearance of the eyelids has improved.

One characteristic of eye-opening correction surgery is that the appearance becomes more natural over time rather than immediately after surgery.
