However, if the columella is underdeveloped from birth, or if it has become contracted and deformed due to complications from rhinoplasty,
there are cases where it is recessed or curled inward.
In such cases, it can be corrected with surgery.

As mentioned earlier, the columella refers to
the pillar between the nostrils.
In the case of the patient above,
cartilage did not develop naturally,
so the columella had receded as shown in the photo.
From the side as well, compared to the tip of the nostril,
the columella had curled inward,
giving a somewhat coarse and old-fashioned impression.


For this patient, ear cartilage was used to extend the recessed lower septal area and correct the columella. If the columella is projected too far downward, it may look like a hooked nose, so it is most important to extend it to an appropriate length.

As in the case above, when the cartilage has not developed or the columella has receded due to contraction,
correction can be performed using autologous cartilage.
For columella projection, artificial implants are not used,
and ear cartilage, septal cartilage, or costal cartilage is used.
