“After the laser, the pigmentation got darker..”
Today, I’d like to introduce a treatment case involving a woman in her 40s who was worried because pigmentation appeared to worsen after a strong pigment laser procedure at another clinic.
She had received Accent and dark spot laser treatments at another clinic.
Although some of the pigmentation did peel off, post-inflammatory hyperpigmentation remained due to the strong laser, so she came to our clinic.
Pigmentation treatment is not something that automatically gets better just by doing it strongly.
If a procedure is performed with excessive energy that does not match the skin condition, post-inflammatory hyperpigmentation (PIH) can occur instead,
and in that case, the pigmentation may appear darker than before, or the treatment period may become longer.
Let’s take a look at the actual process and see how it was treated.

Skin condition before treatment

After diagnosing this patient’s skin,
- Post-inflammatory hyperpigmentation that developed after a strong laser procedure at another clinic
- Existing melasma and dermal pigmentation
- Overall uneven skin tone

This was not simply a case of melasma, but a situation where pigmentation caused by a previous procedure also needed to be improved.
In such cases, using a strong laser again can make things worse, so an approach that minimizes skin irritation and gradually restores the skin was necessary.

Treatment plan and process
Since her skin had already been irritated by a strong laser,
rather than trying to remove pigmentation aggressively, we set a treatment plan that first stabilized the dermal environment and then gradually improved the pigmentation.
With the Triple All For You program, we selected lasers tailored to the skin condition at each visit.
In the early stage, we focused on stabilizing the dermal environment, and in the middle stage, we focused on pigmentation treatment.
Then in the later stage, we monitored the skin’s response to treatment and carried out any additional treatments that were needed.

Early stage treatment (sessions 1–3) - Stabilizing the dermal environment
At first, we combined PicoSure and Genesis Toning to focus on improving the dermal environment.
PicoSure keeps the laser in the skin for an extremely short time, so it causes less damage to surrounding tissue.
That means it can be used relatively safely even on skin that has already been irritated, like in this case.
Also, Genesis gently warms the dermis to induce collagen regeneration and help restore the skin barrier, so it is effective when there is post-inflammatory hyperpigmentation (PIH) caused by a previous procedure.

Middle stage treatment (sessions 4–7) - Focusing on pigmentation treatment
As we continued toning while stabilizing the dermal environment, the pigmentation improved significantly after just three sessions.
After that, we additionally combined PicoSure, Genesis Toning, and Sylfirm PW mode.
Sylfirm PW is a procedure that delivers radiofrequency energy to the dermis through fine needles to stimulate skin regeneration.
Along with improving dermal pigmentation, it also helped improve the scar-like areas caused by pigmentation.

Later stage treatment (sessions 8–10) - Finishing and maintenance
At the 8th session, she returned after four months, and the pigmentation had been well maintained in the meantime.
We proceeded with additional improvement only in the darker areas of the skin tone.
At the 9th session, we checked with Mark-Vu and saw that the epidermal pigmentation had also improved a lot, so we recommended ongoing maintenance care.
She was satisfied with the results and extended the program for 10 sessions.

Changes after treatment

This is the skin condition after 10 treatments over about 10 months.
- The post-procedure pigmentation that developed after the other clinic’s treatment improved noticeably
- The existing melasma and dermal pigmentation also faded significantly
- The scar-like areas caused by pigmentation improved as well
- The overall skin tone became clearer and more even
In particular, this patient had been struggling with pigmentation after a strong laser,
but by minimizing irritation and treating according to her skin condition, the pigmentation recovered and the melasma she had originally been concerned about also improved, leading to a very high level of satisfaction.
Today, we looked through a case study to see how much improvement can be achieved with 10 Jamsil Pico Toning sessions.
Pico toning is a procedure that can be used when the skin is sensitive due to previous laser treatment, like in this case.

However, rather than relying on pico toning alone, it is often more effective to use several lasers together depending on the skin condition.
This patient was treated mainly with pico toning, but Genesis Toning and Sylfirm were also performed together.
Also, pigmentation treatment is not something that gets better just by being stronger.
Excessive procedures that do not match the skin condition can instead cause pigmentation, and in such cases, recovery takes longer.
As shown in this case, accurately diagnosing the skin condition and selecting lasers tailored to each session is the key to safe and effective pigmentation treatment.
