How Can Sunken Acne Scars Be Treated?
"Sunken scars that even makeup can’t cover—
what should be done to improve them?"
Acne scars, especially sunken marks,
are difficult to hide even with makeup,
and they do not naturally recover well
over time.
Although they may seem minor because they remain
after acne has subsided,
they are actually structural changes caused by
damage to the deeper layers of the skin.
Today, let’s look at why sunken scars
do not disappear easily, what approach
is effective, and what the real variables are
that determine scar treatment results.

- Sunken scars: Why don’t they naturally recover?
잠실여드름흉터
The reason marks remain even after acne has
subsided has to do with the area that was damaged.
When acne progresses deeply,
it does not damage only the epidermis;
it also damages the collagen structure in
the dermis beneath it.

The dermis plays a role similar to a framework
that supports the skin’s elasticity and shape.
If the collagen in this area is
broken or missing,
the skin surface appears sunken there,
forming a scar.
The problem is that collagen in the dermis
recovers very slowly on its own.
While the epidermis renews itself roughly every 28 days,
collagen reorganization in the dermis takes
much longer,
and once the structure collapses,
it does not fill itself back in automatically.

That is why the sunken mark remains
even as time passes.
For this reason, unless a method directly reaches
the dermis and induces collagen reorganization,
sunken scars do not change easily.
When looking into treatment for sunken scars,
it is important to check whether the method
goes after collagen regeneration.
- What is the key to scar treatment?
잠실여드름흉터
One commonly used method for treating
sunken scars is Pico Fraxel.
The key feature of Pico Fraxel is that it creates
microscopic vacuoles in the damaged dermis
to stimulate collagen regeneration.

To put it simply:
When laser energy reaches the dermis,
very small empty spaces are created there.
In order to fill these spaces,
the body produces new collagen in that area.
As a result, the previously empty dermis
is refilled with new collagen,
and the sunken marks change in a direction
toward becoming flatter.
There is a research paper on this mechanism,
so let’s take a look.

This study analyzed the depth and distribution
of vacuoles formed in the dermis after
irradiating with a 1064 nm picosecond laser.
The photo below shows stained skin tissue
cut into a cross-section after picosecond laser irradiation.

The upper epidermal layer shows no damage,
while tiny vacuoles have formed in the dermis below.
The researchers reported that new collagen synthesis
was confirmed at the locations where these vacuoles formed.
What is especially important is that the vacuoles
formed beneath the basement membrane
(at a depth of 300–400 μm from the skin surface).
This depth corresponds to the dermal layer
that is mainly damaged by acne scars.
In other words, it acts at the depth of the sunken scar
and can be seen as effective in filling in
the depressed area.

Here, I want to be candid about one thing.
Even if you receive Pico Fraxel treatment,
the effect on acne scars may be minimal.
That is because the results also vary depending on
how the laser energy is distributed.
Picosecond lasers split the beam into several paths
through optical components before delivering it.
However, the energy distribution reaching the dermis
varies depending on the irradiation method.
The study above also compared two methods.

One method concentrates energy in the center
and weakens toward the edges.
In this case, large vacuoles form in the center,
but there is almost no effect at the edges,
so there is a large variation in the area treated
with one procedure (variation of about 26–28%).

The result on the left in the photo above is
this method.
The other method distributes energy evenly
over the entire beam.
In this case, vacuoles form evenly across the dermis
at similar sizes (variation of about 1–5%).
In the end, depending on whether acne scars are
widely spread out or occur one by one,
it is necessary to choose a more suitable method
for better results.
Because of these detailed factors,
there can be differences in effectiveness.
Treatment for 잠실여드름흉터 actually needs to
be carried out over a long period of time.
At this point, treatment must be performed
with a method suited to acne scars in order
to see results.
Even with Pico Fraxel, results can vary depending
on how it is done,
and better results may be seen when it is combined
with other methods that also stimulate collagen regeneration.
So, I hope you will first carefully check
the depth and distribution of your scars.
