This is something we often hear in the consultation room.
Many people visit us because the marks become visible again when the lighting changes, even if they are covered with makeup.

Once a depressed mark has formed, it does not fill in on its own over time.
This is because the indentation is not on the skin’s surface but deeper within the skin.
So, can depressed marks really not become less noticeable?
There is a clinical study related to this, so let’s take a look at it together.

Why don’t depressed marks disappear?
Depressed marks do not disappear because the skin structure has broken down beneath the surface, rather than merely being affected at the surface level.
When acne inflammation is severe, the collagen tissue that supports the inside of the skin can break down and become disrupted.
Ideally, this tissue would fill in evenly as the wound heals. However, areas that have been deeply damaged may regenerate unevenly.

The problem is that this unevenly tangled tissue continues to pull the skin downward.
Because the base remains fixed in a pulled-down position, the depressed shape remains even when new tissue fills in from above.
If the wound is shallow, new tissue may fill it in and make the area flatter.
However, if the damage extends deeply into the skin, the area is filled not with normal tissue but with scar tissue.

That is why, even with time, depressed marks improve on their own only to a certain extent.
Topical ointments and regenerative creams mostly act on the surface, so they cannot loosen the underlying structures that are pulling the skin downward.
This is precisely why depressed marks are difficult to eliminate.

What methods are effective?
As noted above, depressed marks occur when tissue beneath the skin has become attached and is pulling the skin downward, while the indented area is unable to fill in.
Therefore, to see results, both of these issues need to be addressed.
There is more than one approach.
The tissue that has become attached and is pulling the skin can be released, or new tissue can be formed in the depressed area.

One such approach is radiofrequency microneedling.
Fine needles are inserted into the skin to deliver heat.
This heat helps loosen attached tissue and stimulates the formation of new collagen.
It is a method that acts directly beneath the skin by reducing the pulling force and filling in the depressed areas.
A clinical study examining this method is available, so let’s look at it together.

Patients with acne and depressed marks received two treatments at four-week intervals and were then observed for 12 weeks.
The study compared radiofrequency treatment alone with radiofrequency treatment combined with microneedling.
Since the microneedling treatment was more effective in every respect, we will focus on the radiofrequency microneedling results.
When the severity of the scars was scored, the score decreased from 124 to 104, a reduction of approximately 16%.

The changes were particularly noticeable in narrow, deep marks and angular marks.

Can all depressed scars be addressed?
To be honest, it is difficult to eliminate depressed marks completely in a single treatment.
Since their types and depths vary, no single treatment can address them all.

That is why various approaches are used.
The radiofrequency microneedling described above delivers heat beneath the skin to release the attached bands and encourage new collagen formation.
Pico Fraxel, which creates microscopic injuries within the skin to induce collagen regeneration, or Juvelook Volume, which supports skin regeneration, may also be used in combination.

The methods used depend on the type of depressed scar.
For scars with strong downward pulling from the tissue, treatment focused on releasing the attachments may be appropriate. When the depressed area is deep, focusing on stimulating collagen regeneration may help improve the outcome when planning treatment.
For Jamsil acne scars, the key is to first determine what type of mark you have before attempting to fill the depressed area.
