Many people who decide to get orthodontic treatment are most curious about, and at the same time most concerned about, one question: “Do I have to have teeth extracted?” Because the idea of having a tooth removed itself can be frightening, the treatment process may seem inconvenient, and there may be vague worries about having fewer teeth, people are naturally drawn to the term “non-extraction orthodontics.”

- Lack of space in the dental arch
The first criterion is the degree of lack of space in the dental arch. Everyone’s tooth size and jawbone size are different. Some people have larger teeth but a smaller jawbone, leaving too little space for all the teeth to be neatly arranged in their proper positions.
In such cases, if you try to force the teeth into place without extraction, the front teeth may protrude outward, or the entire dental arch may spread unnaturally, which can actually distort the facial shape.
In general, if the orthodontic plan shows a space deficiency of 4–6 mm or more, or if the dental irregularity is severe, creating space through extraction and then aligning the remaining teeth in a balanced way can lead to a much more stable and ideal result.

- Degree of malocclusion
The third factor is the type and severity of the malocclusion. In general, Class I malocclusion (a condition close to normal occlusion) can often be treated without extraction. However, in cases of Class II malocclusion (when the upper teeth are excessively forward) or Class III malocclusion (when the lower teeth protrude beyond the upper teeth), where the positional discrepancy between the teeth is severe, extraction orthodontics may be needed, or in some cases, treatment may need to be combined with orthognathic surgery.

Orthodontic treatment is not just about arranging the teeth; in the end, it is an important process that is directly connected to the patient’s facial image. If extraction orthodontics is forced on someone whose lips are already thin and whose jawline is sharp even before treatment, the lips may be pulled in further and the face may lose volume, creating a look that appears tired or older.
