Hello. We are Merry Plastic Surgery Clinic, pursuing natural and beautiful changes.
If you have an impression that looks especially aged, there can be various causes.
Among them, the effect of the eyes was not small.
For that reason, today, interest in eye-area cosmetic surgery has been rising not only for double eyelid surgery, which increases the exposure of the eyes, but also from younger age groups to middle age.
In particular, when aging begins, the fat under the eyes tends to protrude noticeably, causing what is called an eyebag phenomenon.
Even with careful management, eyebags may require revision surgery, so care must be taken from the first surgery.
Today, we will explain the causes of eyebags and how they can be improved if revision surgery is needed.
What Are Eyebags?

Looking at our eyes, we can see the eyeball, the orbit (eye socket bone), and various nerves and blood vessels distributed there.
At this time, fat tissue is also distributed around the eyes to prevent collision between the eyeball and the orbit.
If for some reason the orbital fat protrudes like a pouch under the eyes and sags, this is called an eyebag.
Causes of Eyebags?

The main causes of eyebags are either a congenital lack of bone supporting the fat, causing the fat under the eyes to protrude downward, or the weakening of the eyebag’s support over time, leading to protrusion.
Depending on the age group, there can be differences in why the symptoms become more noticeable.
✅ 20s: Orbital fat protrudes, creating bulging under-eye area
✅ 30s: Orbital fat gradually protrudes, making the tear trough look hollow
✅ 40s: Orbital fat sags and the tear trough becomes more distinct
✅ 50s and older: The tear trough skin becomes discolored and looks very tired
What Is Eyebag Revision Surgery?

Methods for improving eyebags can largely include under-eye fat repositioning and lower blepharoplasty.
The problem arises when the same issue occurs again even after improvement.
In such cases, revision surgery for eyebags must be considered to help prevent the same problem from recurring.
Under-eye fat repositioning is a surgery that makes an incision in the conjunctiva, removes adhesions in the tear trough, and repositions the fat.
At the same time, the septum can be reinforced so that revision surgery for eyebags is no longer needed, allowing for careful treatment.
Lower blepharoplasty involves making an incision along the lash line, repositioning the fat, and appropriately removing sagging skin and muscle before closing the incision.
When eyebag symptoms and skin sagging are severe, this surgery can offer a more definite corrective effect.
Both surgeries can be expected to be effective in improving recurring eyebags, but the necessary method should be selected after reanalyzing the cause of the first surgery’s failure along with the severity of the symptoms.
