Hello, this is Merry Plastic Surgery Clinic, where meeting us is always a pleasure.
Doctor, my double eyelids have gotten smaller than before.
I often hear that my eyes look tired.
My forehead lines are getting deeper, and it is getting harder to open my eyes.

If you are worried about these things, it may not simply be aging of the eyelids, but rather brow ptosis, where the position of the eyebrows drops.
Today, we will explain the causes of brow sagging and the differences between brow lift surgery and brow thread lifting in an easy and detailed way.

The problem is not the eyelids, but the eyebrows!
When we commonly feel that the eyelids are sagging, in many cases the eyebrows have actually dropped downward and are covering the double eyelids.
This is called “brow ptosis.” The characteristics of brow ptosis are as follows.
The distance between the eyes and eyebrows becomes narrower
The impression looks gloomy or angry
You use your forehead muscles when opening your eyes → increased forehead wrinkles
The double eyelids become hidden or look smaller
Your eyes feel heavier after Botox in the forehead
Could I have brow ptosis? Self-diagnosis!
✅ When the eyes are open, the distance between the eyebrows and eyelashes is 10 mm or less
✅ You keep tensing your forehead when looking in the mirror
✅ Your double eyelids are gradually getting smaller and feel covered
✅ Your upper eyelids have looked puffy, and you have mistaken it for fat
If two or more of the items above apply to you, there is a possibility that you have moderate to severe brow ptosis.
So, how can brow sagging be treated?
① Brow lift surgery (forehead lift surgery): a surgical method that firmly raises a dropped brow
4 to 5 small incisions made endoscopically
The muscles/ligaments are released and the eyebrows are fixed upward
More than 10 mm of brow elevation is possible
Duration: 7 to 8 years or longer
Recommended in cases of severe brow ptosis
✅ Advantages: strong and long-lasting effect, clear improvement in the eye area
⚠️ Disadvantages: surgical burden, concern about scarring, may be excessive for mild cases
② Brow thread lifting: a non-surgical procedure that simply pulls the eyebrows upward
Uses threads designed for thread lifting (ultrafine 21G threads)
Thin threads are inserted into the skin just below the eyebrows
A simple procedure under local anesthesia or with numbing cream
Almost no recovery period, and daily life is possible right away
Duration: 1 to 2 years (varies depending on the thread material)
✅ Advantages: quick procedure, no scars, less burden
⚠️ Disadvantages: shorter duration, may require repetition
Which method is right for me? Let’s summarize it once more in a table.

For special cases, combinations like these are also possible.
-
Brow thread lifting + forehead Botox → addresses forehead wrinkles and brow ptosis at the same time
-
Brow thread lifting → pre-experience before brow lift surgery → can help predict the effect before surgery
-
Upper blepharoplasty + brow lift surgery → combined improvement of eyelid and brow sagging
Rather than choosing double eyelid surgery unconditionally just because the eyelids seem saggy,
you need to comprehensively analyze the distance between the brows and eyes, forehead wrinkles, and the habit of opening your eyes.
If you want to know which surgery/procedure is right for you, we recommend consulting directly with a specialist at a nearby clinic.
