Have you ever felt discomfort after nose surgery as time went by?
Today, I’ll talk about a “contracted nose” and “nose revision surgery in middle age.”
It feels like my nose has become shorter than before.
The nasal tip feels stiff, and even when I smile, it feels awkward.
It was fine after the first surgery, but after a few years, the shape changed.
After nose surgery, problems sometimes arise as time passes, and one representative case is the “contracted nose.” In particular, among people in their 40s to 60s, more and more patients are considering “nose revision surgery” as past surgical side effects and aging overlap.


What is a contracted nose?
A contracted nose refers to a state in which contracture (tissue shrinkage and adhesion) has formed inside the nose, as the name suggests.
After surgery using implants or autologous tissue, if inflammation or excessive scar reaction occurs, the tissue inside the nose becomes hard and shrinks, causing the nose to become shorter, lifted, and stiff.
Typical symptoms of a contracted nose
A upturned nose shape that makes the nose look shorter because the nasal tip rises
The nasal tip does not move well or feels stiff when smiling
The inside of the nose feels hard and pressed; breathing may also be uncomfortable
The nose shape becomes blunter or uneven compared to before
Precautions for nose revision surgery in middle age
In your 20s and 30s, even if you were satisfied with the result after the first surgery, problems can arise over time because skin elasticity and tissue responses change as you age. In particular, implants such as silicone and Gore-Tex, which were used frequently in earlier generations, often cause problems.
In middle age, revision surgery is often needed for the following reasons.

The nose shape becomes abnormally deformed due to contracture

The previous surgical result is not maintained because of sagging skin caused by aging

Breathing discomfort, nasal congestion, and recurrent inflammation
Treatment process for middle-aged contracted nose revision surgery
A contracted nose is not simply a shape issue; it is a situation that requires tissue reconstruction. Therefore, it must be carried out with much greater precision and caution than ordinary rhinoplasty.

Removal of existing implant: The problematic implant is safely removed.

Dissection of adhesions/scar tissue: The firmly attached tissue is separated to create space.

Autologous tissue grafting: Ear cartilage, rib cartilage, and other autologous tissues are used to reconstruct the nasal structure anew.

Stabilization of the nasal tip/columella: The upturned or blunt nasal tip is improved, and the overall structure is adjusted to be natural and stable.

It’s the MERRY Soothing Injection!
After a direct consultation and examination with the chief surgeon, necessary tests such as CT and nasal endoscopy are performed first.
Before the revision surgery, the MERRY Soothing Injection is administered to make the nose literally soft and supple,
and after restoring the stiff contracted nose to a state before the first nose surgery, the contracted nose revision surgery is performed.

This is aimed at making it soft and supple like this.
Usually, about 1 to 2 months after one injection, the nose becomes softer. Depending on the condition, the injection may be given up to about 3 times to help recovery and regeneration before surgery, and the results are much better when surgery is then performed.

Things that were previously impossible have become more improvable as medical techniques have advanced over time.
