Bromhidrosis refers to an unpleasant odor from the armpits, and in Korea it is commonly called "arm odor." This condition is related to sweat glands called apocrine glands.
Apocrine glands are distributed heavily in the armpits and secrete fatty acids and organic substances together. The secreted substances are broken down by bacteria, producing an unpleasant odor.

Bromhidrosis has a strong genetic component, and symptoms often appear after puberty. It occurs more commonly in Westerners than in East Asians, and more often in women than in men.
The main symptoms of bromhidrosis are as follows.
- Unpleasant odor
There is an unpleasant odor from the armpits. The smell varies from person to person and is similar to the smell of onions or rotten eggs.

- Dampness in the armpit area
You sweat a lot and always feel damp. This can cause clothing to become wet or stained.
- Social stress
The odor caused by bromhidrosis can lead to social stress. People may avoid contact with others or feel uncomfortable using public transportation.
Bromhidrosis not only causes inconvenience in daily life but can also affect interpersonal relationships, so active treatment is necessary.

In general, when temperatures rise, sweat production increases and armpit odor becomes worse. However, armpit odor can also worsen in cold weather for the following reasons.
In cold weather, people wear thick clothing, which makes the underarm area become damp easily. This creates an environment where bacteria can thrive, worsening the odor.

In cold weather, indoor activity increases, and as activity levels rise, sweat production can increase. Also, when indoor temperatures rise due to heating, sweating can be stimulated.

During the dry winter season, dead skin cells increase, and when these dead skin cells mix with sweat, an environment that makes it easier for bacteria to grow is created.
If armpit odor becomes worse even in cold weather, it is advisable to improve daily habits and consult a medical professional.

Bromhidrosis may be suspected based on the odor noticed by oneself or others. It can also be suspected if there is a family member with bromhidrosis or if earwax is moist.
In addition, symptoms can be clearly identified through objective diagnostic methods.

-
Odor meter: After wiping away sweat on the armpits, use an odor meter to measure sweat volume and odor.
-
Medical interview: A specialist listens to the patient’s symptoms and diagnoses bromhidrosis.
-
Tissue examination: In severe cases of bromhidrosis, tissue examination can be used to confirm the type and distribution of sweat glands.
If bromhidrosis is suspected, it is important to diagnose it through the above tests and choose the appropriate treatment method for removing sweat odor. Since the appropriate treatment may differ depending on each person’s symptoms and situation, it is advisable to make a decision after sufficient consultation with medical staff.

The sweat odor removal SIT surgery performed at our clinic is a method in which the apocrine glands are cut by about 3 mm and then suctioned.
Because sweat does not occur again from the cut apocrine glands, the key advantage is that the root cause of bromhidrosis can be removed. It is also appealing that the procedure is performed without anesthesia and involves almost no bleeding or pain.
The procedure takes about 30 minutes for both armpits, does not require a separate hospital admission, and allows immediate return to daily life.

Although satisfying changes can be felt after just one session, recurrence rates may vary from person to person, so regular checkups may be necessary.
First, the location of the sweat glands is identified using ultrasound or endoscopy, after which the skin is incised and the sweat glands are removed. At this time, care must be taken not to damage surrounding tissue, and the excised skin is sutured. Afterwards, antibiotics are administered to prevent inflammation at the incision site, and a compression bandage is applied.
Recovery varies from person to person, but generally about one week of recovery is needed. During this period, it is advisable to avoid drinking alcohol or smoking and to get enough rest.

Showering is possible starting on the third day after surgery, but it is recommended to wait two weeks before soaking in water for a long time or using a sauna. Strenuous exercise is also recommended to begin after two weeks.

After sweat odor removal surgery for bromhidrosis, the following care and lifestyle adjustments are needed.
-
Keep the surgical site clean and be careful not to let water touch it.
-
Refrain from drinking alcohol and smoking for a certain period after surgery.
-
Get enough rest and sleep, and manage stress.
-
Reduce high-calorie foods and fat intake, and eat more vegetables and fruit.
-
Eliminate sweat and promote blood circulation through exercise. However, excessive exercise should be avoided for a certain period after surgery.

For patients with bromhidrosis, winter can be another season of suffering. If you are worried about armpit odor, it is a good idea to consult a specialist and find the appropriate sweat odor removal treatment.
