Some people are born with uneven eyes, but it can also happen when the strength used to open the eyes differs, when only one side has a double eyelid, when the shapes of the two eyes differ after eye surgery, or when there is a difference in the epicanthal folds.

If the amount of visible iris is similar but only the size of the double eyelids differs, symmetry can be achieved with double-eyelid correction alone. If there is a difference in upper eyelid hollowness, fat grafting above the eyes is performed. If the double-eyelid height differs, the existing line is released, then a new line is created to match the desired line and corrected accordingly.

Not everyone is a candidate for uneven eye correction. If the difference is not due to the structure of the eyes but instead due to a different bone position, a different angle of the brow bone, a different direction of the bone housing the eyeball, or a different degree of eyeball protrusion, uneven eye correction is difficult.

Many people think that because the eyes are different, maybe only one eye needs surgery. This is possible, but it is not common. If correction is done on only one side, the asymmetry may not be fully corrected, and by identifying various eye shapes before surgery and operating on both sides at the same time, unevenness can be minimized.
