The most common cause may be strong adhesion from fixation that is too deep. This usually occurs after incisional double eyelid surgery. To address this, an incisional revision surgery can be performed to thoroughly release the previously connected deep fixation and adhesions, then re-fix the line with shallower fixation.



Another cause is a sunken scar. In this case, after checking for excess skin, if there is enough skin available, the scar can be removed by excising skin that includes the sunken scar. If there is no excess skin, it may be possible to improve the condition to some extent by loosening the existing fixation as much as possible, though not completely.


In another case, after non-incisional double eyelid surgery, part of the eyelid crease may look pinched. In such cases, improvement is possible by using an incisional revision method to cut the existing fixation and then re-fix it more shallowly.
