When is revision double eyelid surgery needed, and what methods are available?

Hello, I’m Dr. Choi from Wink Plastic Surgery. This time, I’d like to talk about revision double eyelid surgery. After the first surgery, revision surgery is often performed because of dissatisfaction with the result. The most common reason is dissatisfaction with the shape, followed by issues such as the fold becoming loose.

Among cases of dissatisfaction with the shape, if the goal is to make the double eyelid fold larger, this can usually be done with a non-incisional natural adhesion method. However, if the previous scar is severe, or if the previous surgery was done with an incision, an incisional revision may be more appropriate.

Among cases of dissatisfaction with the shape, if the goal is to make the double eyelid fold smaller and lower, this is only possible with the incisional method. Since the existing high double eyelid fold must be released and changed to a lower one, the revision is performed by creating a fold through an incision at the desired height and releasing the existing fold above it.
If the front needs to be lowered and the back raised, or if part of the line shape needs to be raised while another part is lowered, an incision is necessary because the lowering portion is involved.

If you are considering revision surgery because the double eyelid fold has loosened, the surgery should match the cause. If a buried suture method that had remained stable for a long time becomes loose, or if the skin is thin and there is little sagging, revision may be possible with a non-incisional natural adhesion method. However, in most cases of a loosened double eyelid, the incisional method is more appropriate. In particular, when the skin is thick, there is a lot of fat, and the skin is sagging, the incisional method is the way to address this, and because it creates stronger adhesion, it can significantly reduce the chance of the fold loosening again.

Also, depending on the case, procedures such as eye correction for ptosis, ptosis correction, or epicanthoplasty may need to be performed together, so it would be best to decide after consulting with a plastic surgery specialist.
That's all for today’s look at revision double eyelid surgery.
