Many people worry about uneven eyes after double-eyelid surgery, and some are concerned even without having had surgery. So today, I’ll explain this issue in detail.

First, I’ll explain the types of uneven eyes by cause. Broadly, uneven eyes can be divided into those caused by the eyes and the bony structure around the eyes, and those caused by the double eyelids and the eye shape.

Since the eyes are part of the face, when there is facial asymmetry, asymmetry in the eyes and the surrounding bone structure often occurs as well.

Asymmetry of the eyeballs usually refers to differences in the eyes’ own size or angle due to differences in the length or degree of protrusion of the eyeballs. Asymmetry of the bones around the eyes refers mainly to differences in the cheekbones that support the eyeballs and the brow bones that cover the area above the eyes.

Differences between the two eyes caused by asymmetry in the eyeballs and the skeletal structure around them are actually difficult to improve surgically. Some patients wonder whether contour surgery could improve this kind of asymmetry since it is caused by bone, but in conclusion, that approach is risky and essentially difficult.

Then, secondly, what are the types of uneven eyes caused by the double eyelids and eye shape?

First, this may refer to differences in the thickness or shape of the double eyelids when viewed from the front. For example, one side may have a thicker double-eyelid line, or one side may have a thicker front line while the other has a thicker back line—an unbalanced double-eyelid shape.

In terms of eye shape, there are cases where the strength of the eyelids differs between the two eyes, such as with ptosis. In such cases, the muscle strength of one eye may be weaker than the other, making it look sleepier and feel heavier.

However, there are also cases where asymmetry caused by the double eyelids and eye shape exists in combination. In such cases, correcting uneven eyes can often be more difficult surgically, so it is important to choose a clinic and doctor with extensive experience in revision double-eyelid surgery.
