A double eyelid refers to the phenomenon in which, if the skin of the eyelid is connected to or adherent to the tissue inside the skin (the eyelid muscle or eyelid tarsal plate), the eyelid skin folds inward as the eye opens upward. Based on this principle, the shape of the double eyelid is determined by at what height and in what direction it folds.

Double eyelid loosening occurs when the connection or adhesion between the skin and the tissue inside the skin becomes lax.

The most common cause of double eyelid loosening is thick skin and subcutaneous tissue. People with thick skin have strong skin recoil (the force that tries to return the skin to its original, unfolded state), and in the case of people with thick subcutaneous fat layers in the eyelids, this recoil is also strong, causing the double eyelid to loosen.

The second cause is external force, meaning force applied from the outside. Therefore, double eyelid loosening commonly appears in people with allergies or atopic dermatitis, or in people who frequently rub their eyes. It can also appear after swelling becomes severe, such as after bumping the eye or the day after crying heavily. This may be temporary, but repeated loosening often leads to actual loosening.

In addition, surgical methods with weaker fixation, such as non-incisional double eyelid surgery, are more likely to loosen. On the other hand, incisional double eyelid surgery is actually less likely to loosen easily. Therefore, in revision surgery for double eyelid loosening, the incisional method should be considered first.

Double eyelid loosening can also have various other causes, such as eyes that appear sunken due to little fat, or cases accompanied by ptosis, in which the muscles that open the eyes are weak. These causes can also act together.
