Hello, this is Dr. Seung-hyeop Choi, Director of Wink Plastic Surgery. Today’s topic is double eyelid surgery in middle age. As people get older, their eyelids often sag more and more, making it uncomfortable to open their eyes; many also feel that their eyes have become smaller than before. For these reasons, many people plan double eyelid surgery in middle age. In this post, I will explain the points to be careful about and the important factors to consider in such cases.
The most important point in double eyelid surgery for middle-aged patients is naturalness.
In particular, if someone who has never had a double eyelid considers surgery for the first time in middle age, the first concern is often whether it will look unnatural or not suit their face. Therefore, to avoid this, it is important not to make the crease too large or too obvious when performing double eyelid surgery on people with thick skin.

The important point in double eyelid surgery for middle-aged patients is naturalness.
In addition to thick skin, caution is also needed when the distance between the eyebrows and the eyes is narrow, because the thicker the double eyelid crease is, the higher the chance that it will look unnatural.

A thick double eyelid crease can lead to what is commonly called a “sausage eye,” so caution is needed.
For people who already have double eyelids but want to improve sagging, a lifting procedure can be considered first. If forehead sagging is particularly severe, a forehead lift should be considered first. This is because lifting a sagging forehead can relieve eyelid hooding, after which double eyelid surgery can be performed.

If there is eyebrow sagging, a sub-brow lift can be considered.
For cases with less sagging, or when the outer corners of the eyes have mainly drooped, a procedure called a sub-brow lift can improve skin sagging above the eyelid line. Because the incision is made under the eyebrow, the scar can be concealed, and if it is not combined with double eyelid surgery, the recovery period becomes considerably shorter, which is an advantage.

If you are dissatisfied with the shape of your double eyelids, improvement should be done through double eyelid surgery.
Next, if you are dissatisfied with the shape of your double eyelids or if the crease is too small and needs to be raised, it is a good idea to plan double eyelid surgery. In the case of double eyelid surgery for middle-aged patients, while reshaping the crease, sagging skin can also be removed from above the newly created eyelid crease, which can help improve sagging as well.

If middle-aged patients have ptosis, eyelid correction is performed together with double eyelid surgery.
In addition, many middle-aged people have ptosis or sleepy-looking eyes, so eyelid correction is often needed. Unlike simple skin sagging, this refers to cases where the eyes still look sleepy even when open because of loosened eyelid muscles. This is done by tightening the weakened levator muscle, allowing the eyelids to open with more firmness. Therefore, if you are considering double eyelid surgery in middle age, it is effective to consult your doctor about whether eyelid correction is needed and, if necessary, have it performed together.

If the area above the eyes is sunken, fat grafting to the upper eyelid can be considered.
In addition, if the eyelid is hollowed out, upper eyelid fat grafting can be considered, and conversely, if there is a lot of fat in the upper eyelid, fat removal from the upper eyelid can be considered. If the skin at the inner corner of the eye (the epicanthal fold) is excessive, an inner corner incision can also be performed.
