Hello. I’m Choi Seung-hyup, Director of Wink Plastic Surgery. Today’s topic is lower eyelid fat repositioning surgery. When the fat under the eyes protrudes, the area below the eyes looks dark and shadowed, giving a tired impression.
This dullness under the eyes is also called a “dark circle.”
This phenomenon usually occurs because aging loosens the septum that surrounds the fat under the eyes, causing the fat to protrude. However, some younger people naturally have puffy under-eyes from the start. In such cases, because it is not caused by aging, improvement is possible through surgical treatment.

Dark circles can be improved through lower eyelid fat repositioning.
Then, what is the surgical method that can effectively improve this puffy under-eye fat?

If the under-eye area is puffy or dark, it gives a tired impression.
If there is little sagging of the skin under the eyes, the best approach is lower eyelid fat repositioning, which spreads out the excess fat to improve puffiness under the eyes. The surgery usually accesses the fat through a transconjunctival incision, which has the advantage of leaving no scar on the skin. Therefore, lower eyelid fat repositioning surgery involves relatively little bruising or pain after surgery, and recovery is very quick, so returning to daily life is not difficult. For that reason, it is often performed over a weekend.

If there is little skin sagging, lower eyelid fat repositioning surgery is more suitable than lower blepharoplasty.
After that, the adhesions in the under-eye groove are released and the under-eye fat is spread downward and repositioned well, allowing the existing puffiness under the eyes to improve. In addition, if the under-eye area is hollow, the 부족한 volume can also be filled with microfat grafting, so it is sometimes considered together with lower eyelid fat repositioning surgery.

Weakening of the septum due to aging causes under-eye fat protrusion.
Some people are also concerned about recurrence after lower eyelid fat repositioning surgery. However, in the case of lower eyelid fat repositioning surgery, recurrence is very rare, and any later skin sagging caused by aging can be improved by removing the skin.

Lower eyelid fat repositioning surgery can improve puffiness of the under-eye fat.
That concludes our look at lower eyelid fat repositioning surgery. If the under-eye area protrudes puffily and gives a tired impression, lower eyelid fat repositioning surgery can help improve it into a more lively appearance.
