As we age, puffiness, dullness, and sagging under the eyes can make us look older and tired. In such cases, lower eyelid surgery is one option to consider.

Let’s take a look at lower eyelid surgery.
If there is little sagging of the skin under the eyes and the skin still has elasticity, under-eye fat repositioning, which approaches through the lower eyelid conjunctiva and redistributes the fat, can be considered first.
However, if there is sagging skin along with puffiness under the eyes, the loose skin needs to be removed, so lower eyelid surgery should be considered.
Lower eyelid surgery can be effective when the under-eye fat protrudes prominently and when the skin under the eyes has sagged.

When the area under the eyes is puffy and sagging, it can make you look older and tired.
So how is lower eyelid surgery performed?
The surgery begins with an incision made just below the lower eyelash line. Through this incision, the sagging skin under the eyes is removed. Because this area tends to leave scars less easily, the scar is usually not very noticeable after recovery. Through the incision, the area under the eyes is accessed, and the protruding fat is removed and repositioned. The septum, which is the membrane surrounding the fat, is also tightened and reinforced to help prevent recurrence later on. If there is little volume under the eyes or if the area is sunken, fat grafting may also be performed at the same time.

Lower eyelid surgery removes sagging skin and removes or repositions under-eye fat.
So what should be kept in mind during lower eyelid surgery?

Lower eyelid surgery can create a brighter under-eye area.
First, it is best to avoid removing too much skin. If you try to smooth out under-eye wrinkles excessively or remove as much sagging skin as possible, a shortage of under-eye skin can occur, which may lead to ectropion, where the lower eyelid is pulled downward.

If too much skin is removed during lower eyelid surgery, ectropion and other issues can occur.
It is also best not to remove too much under-eye fat. If too much fat is removed, the under-eye area may become hollow instead, which can make you look older, so caution is needed.

If there is hollowing under the eyes during lower eyelid surgery, fat grafting can be considered.
