Hello.
I am Dr. Choi Seung-hyeop, Director of Wink Plastic Surgery.
In the process of considering eye surgery,
“Why is a forehead lift considered together when it is eye surgery?”
“Isn’t a forehead lift a surgery done when you get older?”
Questions like these may arise.
Many people think of a forehead lift simply as a surgery to improve aging,
but a forehead lift plays an important role in making the results of eye surgery more stable
and fundamentally improving the appearance of the eyes.
Today, I will explain in what cases a forehead lift should be considered together with eye surgery.

The cause of droopy eyes is not just a skin problem
Drooping eyelids or a cramped-looking eye shape
do not occur simply because the skin has become loose.
In reality, the following factors act together in combination.
-
Tension and usage habits of the forehead muscles
-
Eyebrow position and left-right balance
-
Function of the muscle that opens the eyes (levator palpebrae superioris)
-
Skin elasticity and thickness of the eyelids, and the degree of tissue sagging
Especially when the eyebrows descend or the forehead muscles are used excessively, the eyes may look smaller than they really are, or give a tired impression.
In such cases, there may be limits to improvement with double eyelid surgery or upper blepharoplasty alone.

Why a forehead lift is needed in middle age
After middle age, not only skin sagging but also changes in muscle function and eyebrow ptosis often appear together.
-
A cramped impression because the distance between the eyes and eyebrows becomes shorter
-
A habit of putting excessive force into the forehead when opening the eyes
-
Increased forehead and glabellar wrinkles
-
A heavy, tired-looking eye shape
In such cases, if only eye surgery is performed, temporary improvement may be possible, but the eyebrow position and muscle-use pattern remain the same, which can limit the extent of improvement.
When a forehead lift is performed at the same time, the eyebrow position and forehead muscle environment are adjusted together, which helps maintain the eye-area improvement more stably.

Cases in which a forehead lift may be needed even in your 20s and 30s
A forehead lift is not necessarily a procedure only for middle-aged or older adults.
In cases where the eyes are naturally deep-set rather than sagging, and the eyebrows are very close to the eyes, a forehead lift may be needed even in your 20s or 30s.
Also, if the following apply, a forehead lift can be useful even for younger people.
-
A lot of fine forehead lines compared with peers
-
An impression that the eyes look covered, cramped, and smaller than their actual size
-
A double eyelid that is not small, but does not show well
-
Putting a lot of force into the forehead and eyebrows when opening the eyes
In cases where the habit of opening the eyes is itself the cause,
eye surgery alone, such as double eyelid surgery, upper blepharoplasty, or ptosis correction, may make it difficult to achieve a fundamental improvement.
By adjusting the structure used to open the eyes through a forehead lift, a more comfortable and natural eye shape can be expected.
Eye surgery is not a surgery that simply looks at the “eyes” alone
Eye surgery is not just a procedure to create a double eyelid line,
but to create a natural and stable result, it is necessary to comprehensively analyze the structure around the eyes, the position of the eyebrows, the pattern of forehead muscle use, and eyelid function.
At Wink Plastic Surgery,
we comprehensively evaluate each person’s facial structure and function to determine whether eye surgery alone is appropriate, or whether it should be combined with a forehead lift.
If you are considering improving the eye area, we recommend an approach that considers not only simple shape or design, but also the anatomical structure around the eyes and the harmony of the entire face.
