In textbook terms, a total of five incisions is standard for a forehead lift. These are positioned to evenly cover the entire forehead, including the center forehead line, above both eyebrows, and the temple areas. However, if the sagging is not severe or the patient is relatively young, surgery may sometimes be performed with only three or four incisions. What matters most is not the number of incisions, but how evenly the entire forehead can be dissected.

In an endoscopic forehead lift, the most important step is precisely treating the corrugator muscle, which pulls the eyebrows downward. If this muscle is appropriately removed, glabellar wrinkles are softened and the inner eyebrows are naturally lifted. There are precautions to keep in mind during this process. First are the nerves that control sensation in the forehead and scalp, and second are the motor nerves that control eyebrow movement. In particular, important motor nerves pass through the area about 1 cm above the eyebrow, so it is very important to dissect while carefully avoiding this region. In addition, large blood vessels pass through this area as well, so damage can lead to a high risk of bleeding. For that reason, the procedure is technically difficult and should be performed by a medical team with extensive experience.

An endoscopic forehead lift is by no means an easy procedure. Rather, it is a high-difficulty surgery that requires sufficient dissection while avoiding nerves and blood vessels. Cases where the eyebrows do not lift enough, or where asymmetry develops on the left and right sides, usually result from insufficient handling of these key areas. Therefore, when consulting about an endoscopic forehead lift, be sure to check not only the scarring or cost, but also whether the surgeon has enough experience with forehead lifts.
