Hello, this is AB Plastic Surgery.
Today's topic is: "I want eyes that are longer from side to side. If I do both inner and outer canthoplasty, will they become like that?"!!
If there is a lot blocking the eyes from the front and back,
removing it can make them look much longer~!!
So, let's take some time to check whether they can be opened up~!!
![[Column] Will It Make a Difference to Do Both Inner and Outer Canthoplasty? image 1](https://pub-9f2bb3498faf4d1d8714b41df24753e3.r2.dev/content/clinics/archive/b8z39rap27/naver_blog/abps20/assets/by_hash/a81423ac6324e86c2b8b44729d727e27955e2a2e890bb62ec5b6d221a0518b83.jpg)
The structure of the eye consists of the inner and outer ligaments that fix the eye socket,
and the orbicularis oculi muscle that surrounds it in a circular shape.
The large eyes of Westerners can be seen as being in a state where they are almost fully exposed, with little being covered~~
However, in East Asian eyes, excessive orbicularis oculi muscle covering the front and upper areas
is often found~~
This is also called an epicanthal fold~~
In other words, since the front is covered,
if that is removed, your naturally larger eyes may be seen more clearly~~!!

There are various methods,
but I brought an illustration that is suitable for understanding the structure.
(Ref: Cometic and structural outcomes of two different techniques of medial epicanthoplasty according to epicanthal fold classification and severity: A cohort study)
If the front area that is covered is cleared away like this, the front becomes longer and looks more open,
so this is called inner canthoplasty.

Then what about outer canthoplasty??
In some cases, there is a particularly large amount of orbicularis oculi muscle at the back,
or the fixation point of the eye ligament is too far from the periosteum.
If that is positioned a little closer to the periosteum,
the back edge of the eye can be pulled a little more~~
This may be a little difficult to understand, but in general:
Inner canthoplasty removes excess skin covering the front of the eye,
and outer canthoplasty opens the outer edge of the eye socket itself
and tightens the fixation so that it becomes longer.

In the end, for people who do not have skin covering the front
and whose back fixation is already in a sufficiently suitable position,
inner and outer canthoplasty will not have much effect~!!!
Then how do you know whether inner canthoplasty will be effective??
This is from an earlier column:
Please refer to this postㅎㅎ

Then how can you tell whether outer canthoplasty will be effective??
It’s simple!
Look in the mirror and pull the back of your eye outward.
If the white of the eye becomes a little more visible at that time,
that means it can naturally be widened to that extent.
If pulling it to the side only creates empty space, then there is nothing more to open.

That concludes our look at inner and outer canthoplasty, which can make the eyes look bigger and longer (especially focusing on outer canthoplasty)~!
Please take care of your health during the cold winter,
and I hope you stay strong with your New Year resolutions.
