Hello, this is AB Plastic Surgery.
Today, together with Director Ma Seong-hwan,
let’s learn about nasal ala reduction.

There are of course cases where nasal ala reduction is needed for a bulbous nose.
But does a bulbous nose usually require nasal ala reduction? That is a mistaken idea.

To briefly explain what a bulbous nose means, it refers to a nose that is overall round in shape with a wide tip.
Most bulbous noses also have a low bridge.
When the bridge is low, raising the bridge naturally lifts the nasal alae as well,
which helps define and narrow the shape of the alae.
So in the case of a low bulbous nose, surgery to raise the tip of the nose often resolves it in many cases.
Nasal ala reduction is recommended when the nose is relatively high but the ala tissue is thick.

Scars can remain after any surgery.
There is probably no doctor who can say, “If you do this procedure, there will absolutely be no scar.”

Nasal ala reduction can be performed by either an incisional method or a non-incisional method.
The non-incisional method is a suture-tie method, and it may be effective when the ala tissue is thin.
The effect varies from patient to patient, but generally, if the ala tissue is thick, the effect may be limited.

For the incisional method, there are approaches that involve incisions on the inside and on the outside.
An internal incision nasal ala reduction can hide scars, but the reduction effect may be relatively small.
If an external incision is made, there is a much higher chance of reducing the alae,
but there is definitely a chance that a scar will remain.
For this reason, when performing nasal ala reduction with an external incision method,
we clearly inform patients that a scar may remain before proceeding.
In most cases, it can be covered with makeup, but when not wearing makeup, the scar may be visible, so you must be aware of this
and proceed with the surgery.

To start with the conclusion, nasal ala reduction is a surgery that removes skin tissue,
so if too much is removed, problems can arise.
There are several cases where the surgical result is unsatisfactory.
The first is when someone wants nostril symmetry through nasal ala reduction.
In such cases, the shape and size of the nostrils may become somewhat symmetrical after surgery,
but the overall nose shape may appear even more asymmetrical.

The first is when someone wants nostril symmetry through nasal ala reduction.
In such cases, the shape and size of the nostrils may become somewhat symmetrical after surgery,
but the overall nose shape may appear even more asymmetrical.
The reason is that if the nostrils are asymmetrical, there is a very high chance that the underlying nasal framework is already asymmetrical.
The framework should first be made symmetrical, and then additional ala tissue should be trimmed to create symmetry.
If only nasal ala reduction is performed, the surgical result may be unsatisfactory.

The second is when too much ala tissue is removed, making breathing difficult and causing the shape to look unnatural.
In this case, once the skin tissue has been removed, it cannot be restored, so your options become much more limited.
Depending on the situation, adjustment through revision surgery may be possible,
but there are certainly cases where there is no solution, so if you are considering nasal ala reduction,
it is better to avoid removing too much ala tissue.
Today, we looked at cases where nasal ala reduction is not needed and cases where it is, as well as scars and side effects.
The skin tissue of the nose ages more slowly than other skin and does not easily gain or lose fat.
That means if tissue is removed, it cannot be naturally restored, so nasal ala reduction should be done only after 충분히 thinking it over,
and I would like to emphasize that rather than only performing nasal ala reduction, it is more important to design the overall nose shape together
and then undergo surgery.
