I wrote this post to answer the questions many of you have been curious about.
Q. If there is a lot of fat in the eyelids, do you have to remove the fat during double eyelid surgery?
AB. If there is a lot of fat in the eyelids, we recommend removing some of the fat. Looking at the structures of the eye, from the outside inward there are the skin, the orbicularis oculi muscle, the septum, and then the orbital fat inside the septum. Orbital fat helps protect the eyeball, but if there is too much of it, the eyelids may look swollen and puffy from the outside. In addition, as people age, the orbital fat near the inner corner of the eye can bulge outward, which can sometimes look unsightly. In these cases, orbital fat can be removed during double eyelid surgery or upper blepharoplasty, so we recommend performing fat removal at the same time. However, if all of the fat is removed, the area may look hollow, or double folds may form as the tissue adheres to the area where the fat was removed, so removing an appropriate amount will lead to better results.
