I wrote this post to provide accurate medical information about gynecomastia.
Last time, there was a male patient with severe gynecomastia, and he was curious about how the surgery is actually performed
and what kind of scars are left, so I thought it would be helpful to look at a severe case together.
Patient case) In a severe case of gynecomastia, how is surgery performed, and what do the scars look like?
First, severe gynecomastia refers to grade 3 to 4 gynecomastia. (If you look at the previous post, there is a classification by stage, so please refer to it.)

Severe gynecomastia of grade 3 or higher
Such grade 3 to 4 gynecomastia inevitably requires treatment for sagging skin, that is, skin excision.
<Skin excision> is ultimately a surgery that leaves scars.
So, in a way, when gynecomastia is mild, at grade 1 to 2, I think it is better to perform surgery if necessary.
At that stage, there is no need for skin excision, and only fat or glandular tissue needs to be properly removed.
Then let’s take a look at an example photo of severe gynecomastia.

Although this is an extremely severe case, when there is significant skin sagging like this, skin excision is definitely necessary.
If the nipple is more than 10 cm below the inframammary fold, this can be considered very severe gynecomastia.
Removing the excess skin here is not simply a matter of cutting it off.
It must be done while preserving the blood vessels going to the nipple. That way, there are no problems with wound healing and recovery is faster.

In the diagram above, the green areas (both dark green and light green) are the areas targeted for liposuction and glandular tissue removal.
And the blue area surrounding the nipple is the area where the blood vessels must be preserved.
For this type of surgery, it is certainly advantageous to be treated by a medical team with sufficient experience in female breast reduction and lift surgery.
Experience in handling flaps and blood vessels, not just simple liposuction, is also a factor that affects the outcome.

An incision is made along the lower chest at the boundary of the pectoralis major muscle so that it is as inconspicuous as possible, and scars usually fade over time.
Postoperative photos may differ from reality due to the shooting environment, lighting, makeup, and other factors.
