Hello, this is AB Plastic Surgery.
Today, together with Dr. Choi Jae-ik,
we will review photos of a female patient in her late 40s who underwent surgery at our AB Plastic Surgery clinic.

The patient came in because she was concerned about her puffy eyelids and hoped to have a natural-looking double eyelid.
Looking at the photos, both eyes are asymmetric. On the left side, a slight hidden double eyelid is visible, and due to the drooping of the eyelid, the outer corner of the eye is barely visible.

On the right eye, the double eyelid is not visible, so a drooping fold above the eye can be seen.
Likewise, there is drooping enough to cover the outer corner of the eye, and the distance between the eye and the eyebrow is quite wide.
This can be seen in cases where the forehead muscles are used a lot to hide the degree of eyelid drooping.

One of the most important factors when deciding on surgery is the thickness of the eyelid skin.
The patient had relatively thick eyelid skin, and a significant amount of fat was confirmed above the eyelids.


On average, eyelid skin is about 0.3 mm thick near the eyelashes and about 1 mm thick near the eyebrows.
When people like this undergo upper blepharoplasty, the skin close to the eyelashes is removed, causing the thicker skin to descend and resulting in a noticeable line after surgery.
Because the patient wanted a natural line, to prevent this we planned to remove skin from the thicker area under the eyebrow and to create the eyelid line using the non-incisional method to correct asymmetry, while also planning fat removal to improve the puffiness.

We also generally view the angle of the outer corners of the eyes as being between 5 and 30 degrees.
In both eyes, the outer corners appear to be raised quite a bit, at over 30 degrees.
After improving the drooping of the outer corners, the eyes may appear harsher, and to prevent that, we planned canthoplasty at the outer and lower corners as well.

There is some bruising and swelling, and typically about 70–80% of the bruising and swelling improves during weeks 1 to 2.

This is the photo taken 4 months after surgery.
Compared with before surgery, the puffy feeling above the eyes and the drooping appear to have improved.
As the asymmetry was corrected, a natural double eyelid line was formed, and through the outer and lower corner procedures, the eyes now have a more open and softer impression.
