Q. Nose tip surgery, alar reduction, and bulbous nose surgery all look similar. Are they different surgeries?

I’ll answer the questions many of you have about rhinoplasty!

Today, I’ve prepared an explanation for a point that may be confusing for those who are concerned about the tip of the nose.

Nose tip surgery, alar reduction, and bulbous nose surgery may sound similar, but they are clearly different procedures.

I’ll explain the differences and what each surgery mainly aims to improve.

The figure above shows what the three procedures mentioned earlier can mainly improve.

-1. "Nose tip surgery" is a procedure that raises the height of the nasal tip. Through an incision inside the nose, it refers to a surgery that mainly raises the tip by tying together the tip cartilage or placing it on top of it, as shown in the image below.

It may sound like a good surgery since the height of the nasal tip can be increased by approaching only from the inside, but the people for whom it is effective are limited, and the degree of improvement is not large. It is mainly effective for people whose tip cartilage is strong and whose skin is thin, and it is not a procedure that produces a large effect for people with a blunt nasal tip and small cartilage.

-2. "Alar reduction" is a procedure that reduces the width of the base of the alar, as shown in the image below, when it is wide or thick, through either an external or internal incision.

There are various methods, but the approach is chosen by considering the width and shape of the alar base, the size of the nostrils, and other factors.

-3. "Bulbous nose surgery" is mainly a procedure to improve the blunt appearance of the nasal tip. In the left image below, you can see thick yellow fat above the tip cartilage. In cases like this, the blunt appearance can be significantly reduced only after physically removing that tissue, as shown in the image on the right below, and then lifting the nasal tip. To improve a bulbous nose, simply raising the tip alone does not restore the overall dimensionality or proportions of the nose, so this procedure is usually performed together with an implant for the nasal bridge to achieve a more comprehensive improvement.
