Q. I’m planning to use donated costal cartilage among the rib cartilage options, but I’ve heard that it droops over time. In that case, would it be better to use autologous rib cartilage?
AB.
Although the word “donated” comes before donated costal cartilage, it is still fundamentally rib cartilage. Because of that, it is a very good material to use relatively easily when the nasal septum is insufficient or, in some cases, when a stronger support structure is needed. However, these days online, people say that donated rib cartilage is often absorbed, causing the nasal tip to droop after surgery and also leading to inflammation easily, but this is somewhat different from the truth. Of course, compared with autologous rib cartilage, the degree of absorption and the likelihood of inflammation with donated rib cartilage are slightly higher, but the difference is extremely minor. In reality, whether the nasal tip droops or inflammation occurs depends more on the surgical process itself, such as how stably the support structure was placed.
In conclusion, both autologous rib cartilage and donated rib cartilage are rib cartilage, and both are good materials for building a strong support structure, so it would be best to choose and use them appropriately according to the situation. If the scar from the rib cartilage harvesting process feels a bit burdensome and you want recovery to be a little faster, donated rib cartilage would be better. In cases such as an inflamed nose or a contracted nose, it would still be better to use your own cartilage, that is, autologous rib cartilage.
