“I want to improve my bulbous nose. I don’t want the tip to be pointed.”

During the consultation, the patient explained the desired direction, and after identifying those needs, we proceeded in a way that reflected that as much as possible.

According to the CT results, the septum was small and deviated, making nose surgery relatively difficult,

and since the patient had a round face shape, adjusting the length and height of the nose while considering the balance of the entire face was the key point.

So we approached it this way.

We improved the flat forehead with forehead fat grafting to create a natural and soft impression.

We created a smooth line from the glabella to the tip of the nose using a nasal bridge implant.

We reinforced the columella in a sturdy and natural way by placing autologous rib cartilage as a support.

We performed tip surgery using tip cartilage suturing + partial removal of soft tissue and septal cartilage,

improving the bulbous nose while completing a tip that was not pointed.

Through lateral nasal bone osteotomy, we corrected the deviated nasal bridge and enhanced the shadow and highlight definition of the bridge from the front.

Although swelling had to be taken into account to some extent because eye surgery and facial contouring surgery were performed together, there was almost no bruising around the nose,

and even with the swelling, the three-dimensional effect was clearly visible from the front.

Also, even from the front, the forehead volume looks refined.

I remember the patient saying that the donor site where the autologous rib cartilage was harvested did not hurt at all, and that they were also satisfied with the nose surgery.

From around the 1-month point, the nose line begins to settle properly.

From the front, side, or 45-degree angle, you can see that the three-dimensional effect has improved.

In addition, the curve flowing from the forehead to the tip of the nose also gives a more refined look.
