This orbital fat is held in place by
the ‘orbital septum.’
In simple terms, the orbital septum is what
keeps the orbital fat from protruding.
But aging weakens the strength
of the orbital septum.

As a result, the membrane that was holding
the fat in place loosens, and the fat protrudes outward.
This causes the skin under the eyes
to bulge outward.
The same applies to the skin and muscles.
As we age, the skin stretches and sags,
and the muscles also lose strength and droop downward.
The reason the skin under the eyes sags
and makes you look older is due to these combined factors.
In other words, simply removing the ‘fat’
alone makes it difficult to achieve
the results you expect.
Therefore, the protruding fat must be
repositioned into the sunken area,
and the ‘orbital septum’ must be firmly secured.

The excess stretched skin needs to be partially removed,
and the muscles need to be reinforced.
That is what lower blepharoplasty is.
The key point is this.

One of the most common side effects of lower blepharoplasty

It refers to a condition in which the eyelid
turns outward.

In simple terms, the lower part of the eye
rolls outward and protrudes.
This can cause a chronic feeling of eye irritation and dryness.
Why does this side effect happen?
It can happen because too much skin was removed,
or because the lower eyelid muscle (orbicularis oculi)
has sagged downward.
That is why it is important to remove the excess
skin under the eyes conservatively,
and suturing to support the sagging muscle
is also necessary.

In addition, reinforcement procedures such as
canthopexy should also be considered.
You may have felt frustrated by the
complex medical explanation.
The key is that to prevent side effects such as
ectropion, structural planning is important.

Incision vs. no-incision?
What are the selection criteria?

incision and no-incision methods.

These are the criteria people use
to choose a clinic.
These are certainly pieces of information

But as a practicing doctor,
I would recommend not judging based only on online information,
and instead visiting the clinic in person
for a detailed examination.
I’d especially recommend visiting at least 3 places.
That is because, by going around and comparing,
you can avoid places that operate with a ‘factory-style system.’

In other words, it means you can find a clinic
that offers a personalized solution for you.
This may not feel very concrete right now
and may sound a bit vague.
However, please keep my advice in mind
and visit at least 3 clinics for consultations.

the more you can prevent regrets after surgery.
