Hello.
I’m Jin-young Lim, the chief director of Piap.
If you are reading this article,
you may have started looking for a plastic surgery clinic
because the area under your eyes has begun to bulge
or sag.
Even if you cover it with makeup,
the dark circles become more pronounced,
and because people keep saying you look tired,
you may be searching for middle-aged eye surgery
at a Jamsil plastic surgery clinic.
But terms like lower blepharoplasty, under-eye fat repositioning, and more..
make things confusing
as you search more and more.
As the chief director of a plastic surgery clinic
who performs middle-aged plastic surgery,
I’ll explain the difference
between these two procedures.
If you’re worried about sagging skin,
please read for just 3 minutes.
You’ll be able to tell
which procedure is right for you.

Lower Blepharoplasty vs. Under-Eye Fat Repositioning: What’s the Difference?

- Under-eye fat repositioning
The fat cushion that protects the eyeball
weakens as we age.
During this process, the fat pushes forward
and appears as a bulge.
At this time, the fat is thinly repositioned
into the tear trough and fixed
so the area becomes flatter.
- Lower blepharoplasty
As we age, not only the skin but also the internal
muscle, meaning the orbicularis oculi muscle, also loosens.
In this case, an incision is made along a 2 mm line
below the lash line, the sagging muscle is removed,
and the remaining fascia is pulled upward.

If wrinkles or sagging are minimal,
lower blepharoplasty alone may be possible..
However, in most cases from the 50s onward,
sagging, puffiness,
and a hollow tear trough are often all present,
so these procedures are usually performed together.
As a plastic surgery director,
the approach I personally pursue is to
match the incision as appropriately as possible
and naturally restore the lively appearance
you had 10 years ago.
As we age, the lower fat pushes forward
and the skin becomes lax.
If these two issues are not addressed at the same time,
it can be difficult to get a satisfying result.
Jamsil Plastic Surgery
Suitable procedures by type

① Fat repositioning only — 30s to 40s
Bulging under the eyes
Deep tear troughs
Shadow-like dark circles
Skin elasticity is still present
② Lower blepharoplasty only — late 40s to 50s
Wrinkled, sagging skin
If you gently lift the skin with your finger,
you look much younger
Sagging and fine wrinkles are the main concerns,
rather than puffiness
③ Both together — most people over 50
Sagging skin + under-eye bulging
- hollow tear troughs all present at once
The entire area under the eyes feels collapsed
when looking in the mirror
A case where lower blepharoplasty alone leaves
the bulging and dark circles unchanged
If you want treatment for sunken skin
at a plastic surgery clinic near Jamsil,
getting an accurate diagnosis is the most important thing.
Because there are two causes,
if you only receive one procedure,
the bulging may remain as it is,
or it may look saggy again over time.
Why the proper extent matters for lower blepharoplasty

Sometimes, even after plastic surgery,
there are cases where the problem recurs.
This is because the muscle was not removed
at the proper extent.
There is an orbicularis oculi muscle around the eye,
and if this muscle does not do its job sufficiently,
the area will sag again.
So after the swelling goes down, starting around 6 months later,
the area begins to gradually sag again.
That is because the muscle that should support it
lacks strength.

For patients over 50 who are highly active outdoors,
the septum is already thin and stretched.
In such cases,
the septum should be reinforced in two layers
and sutured in advance.
That is because the support force can be maintained
even after 6 months,
which reduces the recurrence rate.
That is why, before surgery, I pull the lower eyelid
with my finger.
If it does not bounce back within 1 second,
the patient has almost no elasticity,
so I reduce the amount of tissue removed.
If there is some elasticity,
I perform a more active excision.
Without this process, removing too much can cause ectropion,
where the eyelid turns outward,
and removing too little can make recurrence easier.
Finding this proper balance is important.

I begin by accurately identifying that balance
through one-on-one consultations.
That is because satisfaction depends on
preserving what should be preserved
and making only the appropriate level of incision.
If you are looking into plastic surgery in Jamsil,
please check whether the clinic has this standard.
