As autumn arrives with clearer, higher skies, many people become busy. Keywords such as interviews, employment, study abroad, and exams come up often. One of the surgeries many people inquire about before getting a job or going to an interview is "under-eye fat repositioning." In this post, based on a video by Dr. Shin Jong-in, we will look at the "7 steps of under-eye fat repositioning."

Under-eye Fat Repositioning
Dr. Shin Jong-in is a veteran doctor with 15 years of experience in under-eye fat repositioning, and he presented these 7 steps related to the procedure at a conference. Let’s go through everything from STEP 1 to STEP 7.
STEP 1. Skin incision vs. conjunctival incision
Conjunctival incision: Suitable for younger people who do not need loose skin removed.
Skin incision: A method that removes loose skin, suitable for older people.
STEP 2. Under-eye fat removal vs. repositioning
Removal: There is a high possibility that the tear trough will not be resolved.
Repositioning: By cutting the tear trough ligament and repositioning the fat underneath it, this has the advantage of helping resolve the tear trough.
STEP 3. In front of the orbital septum vs. behind the orbital septum
This is about where to approach for under-eye fat repositioning.
In front of the orbital septum: Approaching from the front tends to yield higher satisfaction with the results.
Behind the orbital septum: There is more damage to the under-eye fat, and fat may leak out, which can make surgery more difficult.
STEP 4. Fixation of under-eye fat
The under-eye fat is fixed to the bony area beneath the eye.
However, because the bone cannot be pierced with a needle, the moved fat is fixed to the periosteum, a hard membrane surrounding the bone. At this point, there are two methods: fixing the fat to the front side of the membrane or to the back side of the membrane.
Both methods have their pros and cons. However, based on the many clinical experiences conducted at Objet Plastic Surgery, the method of fixing it on the inner side of the membrane—that is, placing it deeper than the membrane and fixing it on the inside—has resulted in fewer recurring bulges and longer-lasting effects, so our clinic performs the procedure this way.
STEP 5. Fat movement method vs. membrane movement method for the fat
Rather than saying one is better than the other, the concept here is that both the fat and the membrane surrounding the fat need to be moved. So, rather than choosing one method, our clinic combines the advantages of both and uses both methods.
STEP 6. How to use the tough tissue in the under-eye fat area
The tough tissue near the bulging part of the under-eye fat should be fixed by pulling it toward the bone. This increases support and strengthens the structure, preventing it from being pushed forward and enhancing that force even more. Accordingly, our clinic uses this tough tissue to help strong tissue remain well maintained.
STEP 7. Fixation method
Method of fixing inward so the thread is not visible: Although the surgery is more difficult, it gives the thread enough time to hold the tissue and secure support for a longer period.
Method of pulling the thread out and fixing it externally: The surgery itself is easier, but a drawback is that the needle marks remain for a long time.
Recently, a needle has been introduced that can address these drawbacks, and suturing with it is possible. However, because the number of sutures is limited, and at our clinic we perform repeated suturing by checking the intact tissue, strong periosteum, and the points mentioned above, we use the method of burying the thread inward and suturing.
