Today, with the help of Dr. Shin Jong-in of Obje Plastic Surgery, a master of lower eyelid surgery, we will look at how to reduce under-eye fat bulging and under-eye sagging.
Under-eye fat bulging, under-eye sagging
There are largely two ways to correct these symptoms. Both methods have the same goal of addressing under-eye fat, but the approach differs.
First. An incision method that cuts the skin on the front side of the eyelid
Second. An incision method that cuts the conjunctiva on the back side of the eyelid
So which method is more suitable for me? Many people get confused about this, but there is a way to decide in just 1 second.
It is simply 'age'.
Older patients undergo skin incision, while younger patients do not and instead undergo conjunctival incision. Of course, there are variables depending on the case, but this is the basic way to understand it.
The reason for skin incision
Because the goal is to correct sagging and loosened skin along with bulging under-eye fat, this is mainly performed on people in their late 40s, that is, in their 50s and 60s, whose skin tissue has become loose and lost elasticity.
The reason for fat repositioning
This surgery does not involve the skin.
Also, because it is done only through the inner conjunctiva of the eye, there is no scar left on the skin itself and recovery is very fast. However, since it cannot solve skin-related problems, people in their 20s and 30s who have good skin elasticity but only under-eye fat bulging are the most suitable candidates.
For people in their 50s and 60s
If your skin is very loose and you want
to have surgery with fat repositioning (conjunctival incision),
in such cases, if fat repositioning is performed, the bulging from the fat will disappear, but sagging and wrinkle patterns caused by reduced skin elasticity will remain. In fact, if elasticity decreases slightly, the disappearance of the bulge can create a balloon-deflating effect, which may make fine wrinkles appear more noticeable or leave a lingering droopy feeling due to reduced elasticity. For that reason, unsatisfactory results can occur in many cases, so we provide thorough consultation and explain the limitations.
For people in their 20s and 30s
If your skin sagging is not severe,
but you have many fine wrinkles,
please perform lower blepharoplasty with a skin incision.
Even if surgery is performed in your 20s or 30s, sagging can still appear again when you are in your 50s or 60s after 10 or 20 years. In addition, when considering subtle scar marks, an incision is not really necessary for younger patients, so we counsel them by saying, 'That would be too greedy~.'
