Today, I’d like to explain how to lift a drooping upper eyelid: brow under lift.

Brow Under Lift
This is a procedure that corrects sagging above the eyes by removing skin just below the eyebrow.
In this surgery, the skin of the eyelid is thinnest near the eyelashes and gradually becomes thicker toward the eyebrow. By removing the thick skin just below the eyebrow, the thin eyelid skin can be preserved as much as possible, so the result feels less unnatural after surgery, and the scar is hidden along the lower edge of the eyebrow, making it less noticeable.
Because the procedure is simple, it has the advantage of causing less swelling and allowing for faster recovery compared with upper blepharoplasty.
Candidates who need a brow under lift
The advantage of a brow under lift is that it can naturally address only the sagging skin while keeping the eyelid shape unchanged. Therefore, it can be effective for people who want to keep their original eyes while correcting only the sagging above the eyes.

Limitations of a brow under lift
In cases of ptosis, loosening of the double eyelid fold, or asymmetry
A brow under lift is not suitable because it only lifts sagging tissue.
In these cases, it is necessary to correct ptosis through upper blepharoplasty and make the double eyelids symmetrical.
In cases where there are wrinkles on the bridge of the nose outside the brow area, or severe sagging on the outer side of the eyebrow
A brow under lift only removes skin within the range from the inner to the outer side of the brow, so these cases are not easy to address.
When removing skin, it is cut in a spindle shape. If the shape goes beyond a certain range, the eyelid shape may be altered after surgery, so the amount of skin that can be removed is limited.

In cases where there is a deep hollow just above the eye, or the skin is too thin
If there is a hollow above the eye, removing the skin below the eyebrow may instead cause the thin skin to lift upward, making the hollow worse. In people with very thin skin, wrinkles may form after surgery and the shape may look unusual.
In cases where the distance between the eyes and eyebrows is short
Because the skin below the eyebrow is removed, the distance between the eyes and eyebrows becomes even shorter after surgery, which can create an awkward appearance.
In cases where not only the upper eyelids but also the eyebrows have sagged
In this case, if a brow under lift is performed, the eyebrows may droop even more after surgery, so
A brow under lift is not appropriate. In this situation, it is much more effective to lift the sagging eyebrows together through a forehead-brow lift.
Because each patient’s case is different, the most suitable surgery should be determined through consultation with a specialist, but in general, the guidelines above may be helpful.
