The eye procedures that we receive the most inquiries about at our clinic are
'under-eye fat repositioning', 'lower blepharoplasty', and 'dual lower blepharoplasty'.
However, most patients who visit for Gangnam under-eye surgery consultation do not know the differences between these three procedures.

All three procedures have the same purpose of making the under-eye area smoother, but there are a few differences.
Today, we will take some time to look into this in detail.

To understand this, we first need to look at the structure of the under-eye area.
Under-eye structure
If you look at the area under the eye, the outermost layer is the skin, beneath that are the muscle and orbital septum, and the innermost layer is the fat.
Simply put, under-eye fat repositioning is a procedure that manipulates only the fat and orbital septum located in the innermost layer among the four layers, through a conjunctival incision.
On the other hand, procedures that manipulate all four layers are lower blepharoplasty and dual lower blepharoplasty.
In other words, when the term 'lower blepharoplasty' is used, it means not only manipulating the inner fat and orbital septum, but also making a skin incision and additionally working on the muscle.
So in lower blepharoplasty and dual lower blepharoplasty, under-eye fat repositioning is essentially performed as a basic step.

What is under-eye fat repositioning?
It is a procedure for addressing the bulging under-eye fat that makes the under-eye area look puffy and causes the appearance of under-eye bags.
Gangnam under-eye surgery can be done in two ways: a method that makes a skin incision and a method that does not make a skin incision, instead making a conjunctival incision.
Among these, the method using a conjunctival incision is commonly called 'under-eye fat repositioning'.
Because there is no external incision, recovery is faster and there is less concern about scarring.

So what is the difference between lower blepharoplasty and dual lower blepharoplasty?

This is the part that patients seem to find most confusing.

The first difference between the two procedures is the amount of skin incision.
In lower blepharoplasty, the entire lower eyelid area is incised with a long cut, whereas in dual lower blepharoplasty, only about the outer half is incised.
Naturally, a procedure with less skin incision leaves less scarring and has the advantage of a somewhat faster recovery period.
For that reason, when deciding which Gangnam under-eye surgery procedure to choose, it is best to visit the clinic in person and make the decision after having a thorough consultation.

The second difference is whether a conjunctival incision is made.
In both types of lower blepharoplasty, under-eye fat repositioning is performed as a basic step.
In regular lower blepharoplasty, because the skin incision is large, it is possible to access the under-eye fat through the skin incision, and under-eye fat repositioning is performed through that incision.
However, in dual lower blepharoplasty, because the skin incision is smaller, additional conjunctival incision is used to carry out under-eye fat repositioning.

double lower blepharoplasty > dual lower blepharoplasty > under-eye fat repositioning

How to test under-eye sagging
First, you can check by pulling the under-eye area downward with the 'snapback test'.
For people whose skin elasticity has significantly decreased, when you pull the under-eye area and let go, it will not return immediately and will return to its original position after a few seconds.
The exact standard varies depending on the reference, but in normal cases, it should return to its original state immediately.
If it takes more than 1–2 seconds to return, the elasticity of the under-eye area is reduced, so under-eye fat repositioning alone may be limited.
In that case, lower blepharoplasty or dual lower blepharoplasty, which manipulate the skin and inner muscle of the under-eye area, may be more suitable.
However, this is only a simple at-home test and is by no means absolute, so please treat it as a reference only.
