If Too Much Under-Eye Fat Was Removed and the Area Looks Sunken, How Is It Fixed?

The under-eye area looking sunken can be divided into two cases.

1. When there is little under-eye fat and little volume around the eyes

In these cases, after lower eyelid fat repositioning surgery, the under-eye area is actually more likely to look hollow.

Because there was already very little under-eye fat and little volume, removing and repositioning it leaves too little fat to support the area, which creates this concern.

Patients may think that because the problem was caused by lower eyelid fat repositioning surgery, another surgery will solve it.

However, in this case, revision lower eyelid fat repositioning surgery will not bring back the sunken eye area the way the patient wants.

What looks sunken is the disappearance of the protruding under-eye fat, and in the end, the purpose of lower eyelid fat repositioning has already been achieved.

In this case, rather than revision lower eyelid fat repositioning surgery, the correct correction is to fill the hollow area with fat grafting.

Some patients say that as the conjunctival incision and sutured area heal, they contract and shorten, causing the eyelid to turn inward.

This symptom is called entropion.

If the entropion is not severe, it usually improves naturally over time as the contracted tissue gradually stretches again, so there is usually no major need to worry.

However, in some cases entropion is severe, and in such cases surgical correction such as scar tissue release or dermal grafting may be needed rather than revision lower eyelid fat repositioning surgery.

Problems can also occur when lower eyelid fat repositioning surgery and fat grafting are performed at the same time, namely the under-eye area can look bumpy.
