Under-eye fat repositioning is a procedure needed when there is puffiness under the eyes. However, it is not a procedure that is recommended for everyone simply because the under-eye area looks puffy.
Today, I’ll explain who can and cannot properly see the effects of under-eye fat repositioning, and what procedures may be needed together for those who may not fully benefit from it.
If the under-eye area is puffy, can’t you just remove the fat?

Many people think that if the under-eye area is puffy, all that’s needed is fat removal. They assume that improving the bulge alone will make the under-eye area look clean and refreshed.
Both under-eye fat removal and under-eye fat repositioning have the same goal: reducing puffiness in the under-eye fat. However, even with the same goal, there is a difference in how the two procedures are referred to.

Under-eye fat removal literally removes only the fat, while under-eye fat repositioning redistributes the fat evenly across the uneven under-eye area. Because of this, some people who undergo only fat removal say that while the puffiness improved, the result still feels a little lacking.
That is because as the previously bulging under-eye fat is removed, the hollow or sunken appearance that was not very noticeable before can become more prominent. If you receive repositioning instead of removal, you can avoid that kind of disappointment.

People may think that if fat is protruding under the eyes, it’s enough to simply remove it. But in reality, the reason the under-eye area looks uneven is that protruding areas and sunken areas appear together. That’s why if only fat removal is performed, the sunken areas remain uncorrected, which can leave you feeling that something is still missing.
So if you’re concerned about changes in the under-eye area such as sagging, hollowness, or uneven protruding fat, it is better to go beyond simple fat removal and consider repositioning for a clearer improvement.^^

Who succeeds with under-eye fat repositioning? Who fails?

If someone feels that the under-eye fat is protruding or that the area has become uneven and no longer looks the way it used to, it is common to first consider under-eye fat repositioning. However, not everyone who undergoes under-eye fat repositioning to address protruding under-eye fat is satisfied with the results.
To explain based on the people who come to Obje for under-eye fat repositioning consultations, they can largely be divided into two types:
- People with a large amount of under-eye fat and noticeable puffiness
- People with little facial fat around the eyes but only protruding under-eye fat
Both types come in thinking that because the under-eye fat is bulging, repositioning should solve it. Of course, both can achieve satisfying results if they undergo under-eye fat repositioning.

However, in the case of patients in the second category, they may feel a slight sense of disappointment after under-eye fat repositioning. This disappointment is not because the surgery was performed incorrectly, but because they naturally have a small amount of under-eye fat, which limits how much volume can be created to fill in the area.
At Obje, for patients in the second category as well, we recommend “single fat cell grafting” so they can feel a high level of satisfaction with the effects of under-eye fat repositioning.
For people who have little facial fat around the eyes but protruding under-eye fat, the area may actually feel hollow because there isn’t much fat under the eyes. If the protruding under-eye fat is removed through under-eye fat repositioning, the sunken areas can stand out more.
The core of under-eye fat repositioning is to redistribute the under-eye fat properly so that these sunken areas do not stand out, but because the person is naturally low in fat volume, there are limits to how much the hollow under-eye area can be improved. That’s why for these patients, it is better to consider under-eye fat repositioning together with under-eye fat grafting, or to think about under-eye fat grafting first.
These days, however, the idea that “if the under-eye area is puffy, then under-eye fat repositioning is the answer” has become almost like a formula, so many people come in thinking only about under-eye fat repositioning.
