Hello, this is Obje Plastic Surgery.
When you want to improve the signs of aging under the eyes, the first procedure that comes to mind is likely lower eyelid fat repositioning. However, if you are in your 50s or 60s and the skin and muscles under the eyes have also sagged, lower blepharoplasty is more suitable.
At Obje, we recommend either lower eyelid fat repositioning or lower blepharoplasty depending on the condition of the area under the eyes.

One reason Obje is called a place that is good at under-eye surgery is that we recommend procedures that are appropriate for the patient’s current condition. In addition, it seems we are also recognized as a place that is good at under-eye surgery because, beyond lower eyelid fat repositioning and lower blepharoplasty, we have a special surgical method prepared.
That method is called dual lower blepharoplasty.
If you are considering lower blepharoplasty, you may have heard of “dual lower blepharoplasty” at least once. Dual lower blepharoplasty is Obje’s special lower blepharoplasty method, and many people come to us specifically for it.^^
However, many people do not know in detail how traditional lower blepharoplasty and dual lower blepharoplasty differ. Through today’s post, we will explain in detail what dual lower blepharoplasty is. If you are currently worried about under-eye fat or sagging, please read closely for a moment!
Dual lower blepharoplasty? What is that?
Can’t I just have regular lower blepharoplasty?

To begin with, both dual lower blepharoplasty and traditional lower blepharoplasty are good under-eye procedures. Depending on the case, dual lower blepharoplasty may be recommended as a much more suitable under-eye procedure.
So you may be wondering what kind of procedure dual lower blepharoplasty is.
Dual lower blepharoplasty is a hybrid under-eye procedure that combines only the advantages of lower eyelid fat repositioning and lower blepharoplasty.

Lower eyelid fat repositioning is a procedure that repositions protruding under-eye fat through a conjunctival incision. Because the scar heals quickly and the incision is made on the conjunctiva, which is not easily visible, a quick return to daily life is possible.
However, because the focus of the procedure is on repositioning the under-eye fat, it may be less suitable for people whose under-eye skin and muscles have significantly sagged.
In such cases, lower blepharoplasty is recommended. Lower blepharoplasty removes loose skin and also tightens the muscles, so it can improve the fundamental sagging under the eyes.
However, because the skin under the eyes must be incised to achieve such a strong effect, it is best to fully consider the recovery period.

So at Obje, after much research into whether there was a way to combine only the advantages of these two procedures, we developed “dual lower blepharoplasty.” As a place recognized for being good at under-eye surgery, the satisfaction rate with dual lower blepharoplasty is also quite high.
Dual lower blepharoplasty can offer both of the advantages mentioned above: fast recovery and fundamental improvement of under-eye sagging.

Dual lower blepharoplasty involves repositioning the under-eye fat and then making an incision in the skin under the eyes, as in lower blepharoplasty. The most important point here is that the skin incision range in dual lower blepharoplasty is half that of lower blepharoplasty.
Because the incision range is much smaller, dual lower blepharoplasty can achieve the effects of lower blepharoplasty while allowing recovery as fast as lower eyelid fat repositioning.

Dual lower blepharoplasty really seems good!
Please do dual lower blepharoplasty instead of regular lower blepharoplasty!
As explained above, the advantages of dual lower blepharoplasty are special, so those reading this post may also think that dual lower blepharoplasty is the better procedure.
However, one important point is that no procedure can be said to be absolutely and unconditionally better.

Depending on the patient’s under-eye condition, the appropriate procedure differs between dual lower blepharoplasty and traditional lower blepharoplasty. That is why we do not recommend dual lower blepharoplasty unconditionally as the better option; instead, we recommend the procedure that is most appropriate for the patient’s current condition.
There are clear characteristics in patients for whom dual lower blepharoplasty or traditional lower blepharoplasty is recommended. Let me explain in more detail when dual lower blepharoplasty is suitable and when traditional lower blepharoplasty is suitable!
Cases where traditional lower blepharoplasty is recommended
▶️ People in their 50s to 60s with severe skin laxity
▶️ People with deep under-eye grooves
▶️ People with pronounced under-eye fat bulging
Candidates for dual lower blepharoplasty and traditional lower blepharoplasty are first distinguished by age range. For patients in their 50s to 60s, traditional lower blepharoplasty is suitable in most cases.
This is because as people enter their 50s and 60s, the elasticity of the skin decreases compared with younger age groups, so the under-eye area often sags significantly.
As elasticity weakens, the skin feels more loose, the under-eye fat stands out more prominently, and the tear trough becomes deeper, clearer, and longer.
In such cases, rather than dual lower blepharoplasty, it is more effective to lift the sagging skin and loosened muscles through traditional lower blepharoplasty. With dual lower blepharoplasty, it is somewhat difficult to fully improve severely sagging skin and muscles.
Cases where dual lower blepharoplasty is recommended
▶️ People in their late 30s to 40s who do not have severe skin sagging
▶️ People whose under-eye fat and tear troughs are visible, but only slightly so
Therefore, dual lower blepharoplasty is mainly recommended for patients in their late 30s to 40s whose skin elasticity has not yet significantly decreased.
Because the degree of skin aging is not as severe as in patients in their 50s to 60s, under-eye sagging does not appear as prominently. The tear trough may be slightly visible, but it does not feel deep.
For these people, traditional lower blepharoplasty may feel a bit early. However, doing only lower eyelid fat repositioning and stopping there may feel somewhat unsatisfying.

So in such cases, receiving dual lower blepharoplasty can produce the best results.
If dual lower blepharoplasty is used to reposition the under-eye fat, remove part of the excess skin, and tighten the sagging muscles, patient satisfaction may be higher than with lower eyelid fat repositioning alone.
