Crooked Nose That Can Be Corrected vs. Crooked Nose That Cannot Be Corrected

The human face can be divided into the upper, middle, and lower parts. Here, the upper part refers to the forehead and eyes, the middle part refers to the nose, and the lower part refers to the lips and chin.
If the structures in the upper and lower parts are not aligned in a straight line and are out of place, the nose in the middle naturally becomes crooked as well.
In this situation, because the face itself is asymmetrical, it is difficult to correct only the nose.
To improve the underlying cause of a crooked nose, surgery such as orthognathic surgery that aligns the asymmetry of the entire face is needed.

So, what kind of crooked nose can be corrected?
A correctable crooked nose is a case where the structures above and below are aligned, but only the nose in the middle is crooked. It is not the entire face that is asymmetrical, only the nose in the middle is crooked, so it can be corrected relatively easily.
Why Different Surgical Methods Must Be Applied to Different Crooked Noses
There are three main cases where only the nose is crooked, as mentioned above. I’ll explain in detail what those cases are and how surgery is performed.
- When the nasal bone is crooked due to injury or an accident

Some people injure their nose during sports or due to an accident, and as a result, the nose becomes crooked. In these cases, the crooked nose can be corrected through surgery.

When the nasal bone is crooked due to injury or an accident, the bone is first osteotomized, then pushed in the direction opposite to the crooked side to correct it naturally.
For example, if a patient’s nose is crooked to the right, it can be corrected by osteotomizing the bone and then pushing it to the left.

However, if the nose is severely crooked, then medial osteotomy (the inner part of the nasal bone, closer to the center), lateral osteotomy (the outer part of the nasal bone, the area near the eyes to the cheekbones), and intermediate osteotomy (the midpoint between the medial and lateral areas) may all be necessary.
But the more osteotomies are performed, the greater the burden on the patient and the more pain they may experience. Therefore, unless it is truly necessary, Dr. Lee Kyung-mook corrects crooked noses using only medial and lateral osteotomy.
- When the septal cartilage is crooked

When osteotomy is performed to correct a crooked nose, the septal cartilage inside the nose is also affected. Septal cartilage refers to the cartilage between the nostrils that divides the inside of the nose into left and right sides.
Among patients who undergo surgery at a place known for crooked nose surgery, some have septal cartilage that is itself crooked. If the septal cartilage itself is crooked, even after osteotomy and pushing it in the opposite direction, the septal cartilage does not follow. Even if it does, the effect is minimal.

In this case, correction is difficult with osteotomy alone, and an additional procedure called spreader grafting is needed. Spreader grafting is a surgery in which ear cartilage or rib cartilage is grafted onto the side opposite the direction in which the septal cartilage is crooked.
If one side of the septum is hollowed out, the nose can become crooked in that direction. So you can think of this as a method that fills the empty area and helps prevent bending.

Even when he is not performing crooked nose surgery, Dr. Lee Kyung-mook finishes rhinoplasty by filling cartilage into the area if one side of the septum is sunken. At the final stage of rhinoplasty, he carefully checks whether the nose is crooked or whether there are any sunken areas before completing the procedure.

- When the septal cartilage is severely crooked

Although it is not common, among patients who visit for crooked nose correction, there are some whose septum is completely rotated. This is not just a slight bend; the septum is so severely crooked that correction cannot be achieved simply by spreader grafting or by filling in sunken areas with cartilage.
In these cases, surgery called swing door is needed to correct the severely crooked septum.

Swing door refers to a procedure that moves the position of the septum itself. It is not applied to all patients with crooked noses; as mentioned above, it is recommended only for patients whose septal cartilage is severely crooked.
