At Obje Plastic Surgery, we see many patients who visit for eyelid surgery consultations. There are many reasons people want eyelid surgery: some want to look prettier or more refined, while others who are a bit older often come in because they want to make their eyes look clearer and younger.
However, there is something you should think about before having eyelid surgery.

These are the conditions for pretty eyes, attractive eyes, and clear eyes. At Obje, we believe there are four conditions that must be met in order to achieve pretty, attractive, and clear-looking eyes.
In today’s post, we’ll take a detailed look at these four eyelid surgery precautions, so if you are interested in eyelid surgery, please read along.
Do pretty eyes and handsome eyes have conditions too?
Four conditions for a clear-looking eye shape

Although the ideal eye shape may differ slightly from patient to patient, the ideal eyes most people describe are large and open-looking eyes.
Because of that, many people also want their eyes to look bigger and more open when they get eyelid surgery.
At Obje, we also identify four conditions that make eyes look beautiful.

- Eyes with a long horizontal length
- Eyes with strong eye-opening strength
- Eyes with large black pupils
- Eyes with slim eyelid volume
In particular, because the functional aspect of the eyes is also very important, Obje includes strong eye-opening strength when discussing eyelid surgery precautions. I’ll explain each one in detail.
Eyes that are successful for eyelid surgery?
- Eyes with a long horizontal length

The first condition for beautiful eyes is that the horizontal length must be long.
When the horizontal length is long, the eyes can look clear and open whether the double eyelid line is thin or thick. If the horizontal length of the eyes is short, even a slightly thicker double eyelid can make the eyes look like sausage eyes or seem narrow and congested.
That is why the eyes need to have a refreshingly long horizontal length before the double eyelid line can open up to create beautiful eyes.
In fact, not everyone becomes more beautiful just by getting double eyelid surgery. For some people, double eyelid surgery alone makes them look better, while others may feel dissatisfied when only the double eyelids are done.
At Obje, there may be various reasons, but we believe the most important one is horizontal length. Eyes need a long horizontal length to look beautiful, which is why corner-surgery procedures are performed to address this.

The main procedures that open up the eyes are the inner corner procedure, outer corner procedure, and lower corner procedure. Recently, upper corner procedures are also common, but I’ll focus on the three most common corner procedures.

First, the inner corner procedure is mainly performed on East Asian patients who have an epicanthic fold.
If there is an epicanthic fold, the inner corner of the eye feels blocked and gives a cramped impression. In this case, performing an inner corner procedure improves the epicanthic fold, opens up the inner corner, and helps secure more horizontal length.

That said, opening the inner corner too much is not necessarily better.
If you look at the inner corner of the eye, you can see the red mucosa. This area is called the lacrimal caruncle. If the inner corner procedure is done excessively, too much of the lacrimal caruncle may be exposed, which can make the eye shape feel heavy.

At Obje Plastic Surgery, we perform the inner corner procedure conservatively to prevent excessive exposure of the lacrimal caruncle. When doing this procedure for the first time, we always recommend opening it naturally rather than too aggressively.
In fact, the inner corner procedure itself is not a difficult surgery from the perspective of a board-certified plastic surgeon. So instead of doing it aggressively from the start for a dramatic look, it is possible to try a natural result first and then correct it later if it feels a little lacking or if you want it opened slightly more.

However, if the inner corner is opened too much at once, too much of the lacrimal caruncle may become visible, and you may end up needing revision surgery to restore the inner corner. Revision surgery for the inner corner is costly, and it can also leave significant scarring.
For that reason, Obje recommends being conservative with the inner corner procedure as an eyelid surgery precaution.

Second, the outer corner procedure is recommended for people whose interocular distance may become too narrow if the inner corner is opened.
However, the outer corner procedure is actually a surgery that only certain people are suited for. Among people who want the outer corner procedure, 6 out of 10 are often not suitable candidates.

So, when evaluating whether the outer corner procedure is appropriate, we must pull the eye outward and see how much space there is. Some people will show only a small amount of red mucosa at the outer corner in this position. In such cases, there is not enough space at the outer side of the eye, so the outer corner procedure would be difficult.

So what kind of features make someone suitable for the outer corner procedure? If, when the eye is pulled outward, only the white part of the eye is visible, the outer corner procedure is usually possible.
Many people think they must do both the inner and outer corner procedures in order to extend the horizontal length of the eyes, but there is no absolute rule or method for making eyes look beautiful. To create beautiful eyes, you need the right procedure suited to your own eye shape.
That is why Obje Plastic Surgery does not recommend the outer corner procedure for cases where it is not suitable. Also, just like the inner corner procedure, we perform the outer corner procedure without overdoing it, within a range that can make the eyes look larger or more natural.

Third, the lower corner procedure is a corner-surgery procedure that creates a gentle, puppy-like eye shape.
Among celebrities whose eyes we often think are large and beautiful, there are Lee Min-jung and Tzuyu. Their eyes share a common feature: the outer space of the eyes is very wide.
When the outer space of the eyes is wide, the horizontal length of the eyes becomes longer, and the overall eye shape looks clearer. The procedure that creates this effect is the lower corner procedure.

So can everyone have the lower corner procedure? That is not the case.
If, when looking straight ahead, you can see the white of the eye below the black pupil, we do not recommend the lower corner procedure. For people with this feature, the lower corner procedure can make the condition look more pronounced, so we do not advise it.

2. Eyes with strong eye-opening strength

Now I’d like to explain why strong eye-opening strength is important.
Whether or not a person has double eyelids can affect the perceived size of the eyes, but if the eye-opening strength is weak, the eyes can also appear small. That is why, during eyelid surgery consultations at Obje, we take time to assess the patient’s eye-opening strength.

At this stage, we determine whether it is true ptosis or pseudoptosis.
The method for checking whether it is true or pseudo is simpler than you might think. If, when we make a slight double eyelid using a toothpick, the entire pupil becomes visible and the eyes look twice as large, it is pseudoptosis.

With true ptosis, even if we create a double eyelid using a toothpick, the eyes still do not open well. And when we tell the patient to open their eyes wider, they end up using the forehead or eyebrow muscles to do so. This happens because the eye-opening strength is weak.
For these patients, double eyelid surgery alone is not enough to improve the eyes, and ptosis correction is needed. However, as an eyelid surgery precaution, we always say that overdoing ptosis correction is also not good.

If ptosis correction is overdone, the eyes may not close properly, causing dryness or even making it hard to sleep. It is also very difficult to reverse.

Sometimes, after ptosis correction, people feel that their eyelids have become thick. This can also happen when eye-opening strength improves.
The reason is that the eyelids are no longer being pulled upward with the help of the eyebrows or forehead, and instead the eyes are opening using only their own muscle strength, which changes how much the eyelids are pulled.
If you open your eyes using forehead strength, the eyelids are pulled upward more. But after ptosis correction, since the eyelids are no longer being lifted by the eyebrows or forehead, the eyelids may seem somewhat thicker.

This thick-looking eyelid condition after ptosis correction can be improved sufficiently. At Obje Plastic Surgery, we use forehead lift to improve the appearance of thick-looking eyelids.
A forehead lift not only lifts a sagging forehead but also lifts the drooping eyebrows, which can improve the appearance of heavy eyelids. So when the eyelids feel thick, a forehead lift can be a good alternative.

Some people want to do ptosis correction and also make the double eyelid line much larger at the same time. In reality, it is not easy to achieve both at once. Since ptosis correction is a surgery that helps the eyes open better, the double eyelid line may become thinner after the procedure.
That is why it is good to set priorities between ptosis correction and the double eyelid line. If you decide whether improving eye-opening strength is more important or creating your desired double eyelid line is more important, you can get a more satisfying eyelid surgery result.

3. Eyes with large black pupils

I mentioned that the size of the black pupil is important, but in fact this is difficult to address medically. It is hard to enlarge the black pupil through plastic surgery.
Still, there are several alternatives we can consider to improve the appearance of the pupils. A representative option is to use contact lenses such as circle lenses that make the pupils look larger.
Another option is to improve it to some extent through makeup, so even if the black pupil is small, you can still create a beautiful eye shape after eyelid surgery.

4. Eyes with slim eyelid volume

When you look at celebrities who are commonly thought to have beautiful eyes, most of them have slim eyelid volume. There are almost no cases where the upper eyelid area is narrow or very thick.

That is why many patients who come in for eyelid surgery consultations say they are worried about thick eyelids. In such cases, the thickness of the eyelid area can be improved to some extent by removing fat from the upper eyelid.
However, whether the eyelid is really thick because of fat can only be confirmed during surgery.

The eyelid is made up not only of fat, but also of muscle and a structure called the septum. Because of that, simply removing fat does not always solve thick eyelids.
Of course, removing upper eyelid fat may reduce some volume and make the area slimmer, but if there is not much fat or the muscles are thick, the improvement may not be significant.

So at Obje Plastic Surgery, we honestly explain that removing fat does not automatically make the eyes slim. Instead, we suggest other options.
