Hello, this is Obje Plastic Surgery.
These days, many people are worried after hearing that they look tired because of puffy under-eyes
or sagging under-eyes.
That is why many people look into eye-area procedures.
Among them, lower blepharoplasty is
one of the representative eye-area improvement surgeries
drawing attention for its clear effect.
But by what mechanism does lower blepharoplasty
make the under-eye area look younger?
This is the part many people are
most curious about.
Today, to answer that question,
we will first look at the aging process around the eyes,
and then explain step by step
how lower blepharoplasty reverses it.
The eye area that looks aged — is the cause ○○?
50s lower blepharoplasty review, want to hear that you look younger?

- Decrease in collagen and elastin
As we age, various tissue changes occur
in our faces.
When we are young, there is enough
collagen and elastin in the skin,
so the skin stays firm and taut.
These components are proteins that play an important role
in maintaining the skin’s structure and elasticity.

But as we get older,
the amount of these elastic proteins produced
gradually decreases.
Skin that has lost elasticity
becomes weaker at resisting gravity,
so it starts to sag and loosen downward.
As a result, the skin stretches more than before
and gathers in one area.
In simple terms, the skin itself remains,
but because its ability to hold its position decreases,
the excess skin overlaps.
This overlapping skin creates fine lines and deep wrinkles,
making the eye area look even more aged.
- Fat loss
In addition, the fat beneath the skin also changes.
When we are young, the face has a layer of fat spread evenly
under the skin,
creating full and soft curves.

But as we age,
this fat is gradually absorbed and reduced.
Previously, the fat was thick enough to hide
the shape of the bones underneath,
but once the fat decreases, the shape of the bones below
becomes visible through the skin.
That is why hollow or protruding areas such as
the cheekbones and temples become more noticeable.
The face looks much more angular than before,
and the bone structure becomes more defined.
But there is an interesting point here.
Even though fat is said to decrease,
the under-eye area can look puffy and protruding.

In fact, this does not mean the under-eye fat itself has increased.
There is originally fat around the eyes that acts as a cushion
to protect the eyeball.
Under the eyes, there is a membrane called the orbital septum,
which is like a pouch containing fat.

As we age, this septum also loses elasticity and stretches,
and as a result, it can no longer hold the fat properly.
Just as a stretched rubber band can no longer
grasp an object properly,
the septum can no longer hold the fat in place.
Then the fat beneath the eyeball pushes through the septum
and protrudes forward and downward,
causing the under-eye fat pouch to bulge.
- A border caused by fixation of the retaining ligaments
There is another feature of eye-area aging.
It is the issue of a structure called the retaining ligaments.

You can think of the retaining ligaments as a kind of fastening strap
that connects the bones, skin, and muscles,
helping facial tissues stay in place.
As we age, the skin loses elasticity and stretches,
but the retaining ligaments remain firmly anchored to the bones like nails,
so they do not stretch.
Then only the area with the retaining ligaments
is held to the bone,
while the surrounding tissues sag downward,
creating a boundary.
This is the reason for the distinct grooves
such as the tear trough or Indian bands.

In this way, eye-area aging occurs
as multiple tissues change together.
Lower blepharoplasty — a procedure that actually reverses it?
50s lower blepharoplasty review, want to hear that you look younger?
So far, we have looked at the aging process around the eyes.
Now it is time to see how lower blepharoplasty
reverses it.
The first thing to do is
to release the retaining ligaments that are firmly attached to the bone
and pulling the surrounding tissues downward.

If you try to lift other tissues upward while leaving these retaining ligaments in place,
they will still pull downward,
so the tissues cannot move freely.
That is why these retaining ligaments are accurately identified and released.
Then the sagging tissues can move freely
back to their original position,
and the hollow-looking areas can be smoothed out.
Once the tissues are released,
the septal tissue that was supporting the inside is then整理ed.
The problem was that the septum had stretched,
allowing the under-eye fat to protrude forward, right?
So after appropriately tightening the septum,
it is firmly fixed to the periosteum or nearby firm tissue.

If it is only tightened without fixation,
it may stretch again over time,
so this helps prevent that.
Next comes the step of properly addressing the bulging under-eye fat.
When we explained eye-area aging earlier,
we said that fat loss creates hollow areas, right?
In particular, the area below the eyes often becomes hollow
as fat decreases.
But right above that, there is bulging fat.
So the protruding fat is moved into the hollow area
and spread out.

This means taking from where there is too much
and filling where there is not enough.
This creates a smoother,
more natural curve across the under-eye area.
The muscles also lose elasticity as we age
and start to sag downward.
Therefore, the sagging muscle is lifted back
to its original position,
and then fixed to the periosteum or surrounding firm tissue.

The important thing here is to fix the muscle with
the proper degree of tension.
If it is fixed too loosely or too tightly,
it may sag again or cause side effects.
Finally, the stretched skin is carefully removed
by the necessary amount.
Even if all the inner tissues have returned to place,
if only the skin remains stretched,
the skin will still look saggy
and the wrinkles will remain.
So the appropriate amount is removed,
taking into account the patient’s skin thickness,
elasticity, and degree of sagging.
To summarize:
release retaining ligaments – organize septal tissue –
under-eye fat repositioning – muscle fixation – skin excision
Through these steps,
the skin becomes firm again,
wrinkles are naturally smoothed out,
and the entire eye area looks much younger.
So far, we have looked at why lower blepharoplasty is excellent
for a youthful effect, focusing on the underlying principles.
Lower blepharoplasty is not simply a surgery that makes a small incision in the skin;
it is a procedure that reviews and reverses the causes of eye-area aging.
That is why the changes after surgery are clear,
and why it is often said to make people look naturally younger.
Meeting people who are distressed by sagging under-eyes,
I also felt uneasy.
My hope is that you can quickly regain a lively daily life
through accurate diagnosis and treatment.
In particular, because the eye area varies greatly from person to person,
it is most important to find a medical team
with extensive procedural experience that can carefully assess your condition
and provide safe treatment under a systematic plan.
In this regard, Obje Plastic Surgery
conducts systematic surgery tailored to the patient’s facial structure
and aging pattern, based on accumulated procedural experience and expertise.
We hope this has been of some help
in your future decision-making,
and we wish that your careful consideration leads to a satisfying result.

This has been Obje Plastic Surgery. Thank you.
